Prolonged VA #ECMO after cardiac surgery, PELS-1 cohort study, investigating outcomes ??2K adults included from 34 centers/16 countries, 2000-2020 ??support duration 0-3 days = 32.1%, 4-7 = 38.3%, 8-10 = 13%, >10 = 16.5%; nearly 30% of patients supported for > 7 days ?long-term post-discharge prognosis comparable to PC ECMO patients with shorter run ?? U-shape curve of mortality & #ECLS duration observed in this cohort: lowest morality if run duration of 4–7 days, higher if > 10 days; longer run (> 7d) also associated with complications like bleeding, AKI, #sepsis, MOF, but postdischarge survival good. Decision to continue vs cease PC ECMO complex: critical reevaluation should be performed after ± 7 days, and multidisciplinary teams should take age, type of surgery, comorbidities, complications (all more often described in nonsurvivors), and patient values into account. What if the world's 1st?positive clinical trial for hemorrhagic stroke was not only better for patients' functional improvement, but also significantly reduced hospital and healthcare costs? In a late-breaking poster at the International Stroke Conference in Phoenix today, data from the #ENRICH clinical trial demonstrated the cost effectiveness of intervening early to treat this most deadly, debilitating and costly form of stroke when using #MIPS with NICO technologies. ? We were thrilled to see this data showcased from Janel Hammer, MD, PhD, associate professor of medicine and medical director of the Patient Reported Outcomes Center at the University of Pittsburgh Medical Center. Her analyses demonstrated that ENRICH using minimally invasive parafascicular surgery (MIPS) with NICO #Brainpath and #Myriad is a cost-effective approach to improve outcomes in #ICH when compared to standard medical management. This unusual combination of reduced hospital and healthcare costs with improved patient outcomes offers an exciting paradigm shift in how hemorrhagic stroke can be treated in appropriate patients. ? Hemorrhagic stroke costs the U.S. healthcare system approximately $17.2 billion annually.?Among survivors, the estimated annual costs of non-hospital care and productivity losses is about $12.7 billion. We hypothesized that certain echocardiographic variables contribute to the identification of systemic right ventricular failure and accurately predict prognosis in follow-up of these patients. Perfusion, which preserves organs outside the body, is changing every aspect of the organ transplant process, from the way surgeons operate, to the types of patients who can donate organs and the outcomes for recipients.
