***?Stepwise DK-Nanocrush in NSTEMI Patient, Educational and Practical *** 65y old gentleman PMH: HTN ,Non-Diabetic Presented with New Onset Resting Angina ECG showed T wave inversion at lateral leads TTE demonstrated?LVEF:55-60%? Lab tests: Positive cardiac troponin Planned for Invasive strategy Radial SCA Revealed SVD, LAD/Diagonal bifurcation lesion, 0-1-1? 2 Stents strategy was considered based on large well developoed diagonal based on literature, DK-Crush technique? Despite its complexity, is associated with fewer MACE. This case was performed using DK-Nancrush technique in stepwise manner. Nice result TIMI flow: 3 No Complication I have conducted more than 340 Bifurcation PCI cases, most of them carried out using provisonal approach and only 11 cases needed two-stetns strategy. A truly interesting Mitraclip case (very complex challenging for M-TEER) that we did last Friday. A lady with cardiac sarcoidosis on immunosuppressants with ICD implantation presented with worsening dyspnoea. We then noted severe functional MR. She had a LVEF of 25% (not improving) and already on optimized GDMT. TEE showed a cleft at the posterior mitral valve. Discussed in Heart team and proceeded for mitraclip implantation which was successful.
