How it started. My 22-year journey in the nursing field. I started my Nursing career in Surgical Oncology as an LPN and evolved into entrepreneurship in Care Management. This transition from hands-on clinical roles to establishing my own business was not only a professional shift but a strategic move to increase my impact as a #nurse through innovative #caremanagement practices. Initially, becoming a nurse wasn't in my plans, it was an idea suggested by my ex-husband, but this unexpected path has been the most rewarding endeavor of my life, filled with profound personal growth and lasting memories. Nursing has given me invaluable experiences, shaping my understanding of healthcare, patient advocacy, and the systemic challenges within our medical systems. My journey into entrepreneurship came from a desire to harness my clinical expertise and passion for patient care more broadly. I now assist healthcare organizations and fellow nurses in implementing successful care management programs that focus on improving patient health outcomes, reducing hospitalizations, and increasing revenue. A significant part of my mission is to tackle the disparities in healthcare, particularly affecting minority communities, drawing from my own experiences with multiple chronic conditions. As we celebrate Nurses Week, I reflect on the incredible journey this profession has offered me, the challenges I've overcome and the opportunities I have to make substantial impact in the lives of many. Research Opportunity – Recruiting Nurse Volunteers to Participate in a Research Study: Double-Duty Caregivers’ Satisfaction When Communicating with Health Care Providers: A Descriptive Correlational Study ? Hello, my name is Jean Marie Gouveia. I am a nurse and a PhD student researcher at the University of Massachusetts Dartmouth College of Nursing and Health Sciences. I am interested in the experience of nurses who are caregivers in their personal lives for an adult relative or friend. This is also called being a double-duty caregiver (DDC). The purpose of this study is to identify whether DDCs are satisfied when communicating with health care providers, and how their caregiver experience, stress, personal and professional attributes, caring ideals, trust, and intentional communication strategies are related. Better understanding of what this experience is like will help the healthcare team provide improved care to others like you. More information is in the attached document. A system is not the pieces of the system, but the interaction of those pieces. Still, working on a relationship seems more about working on emotional issues, not the interactions of two minds (both systems). This is where I probable get myself in the most trouble--speaking of the mind as a system--which it clearly is--but so what! I've become good at spitting in the wind on this subject. As long as I'm already up to my neck in emotional quicksand, I might as well wiggle deeper. More Dibbleisms: Reality is systematic, created inside/out. Our relationships with the opposite sex, for instance, are created by the relationships we have between our Masculine and Feminine Minds. If we want to heal a relationship, we must heal the miscommunication between these two parts of ourselves AND change the habits (systems) in the outside world that are fear and separation-based. We know we can't solve problems with the same thinking used to create them. More importantly, we can't solve problems with the same emotions that define those problems. How do we heal? We must learn to proactively transform thoughts defined by fear to ones based in love. This is the Mastery of Awareness and the Mastery of Transformation. This is true mastery of relationships and life, yes? It's heartening so many responses to Irith's post below - about a tense physician-nurse interaction - espouse truth, trust, and psychological safety for all team members. TRUTH: a similar experience taught me healthcare selects for traumatized people with lots of ACEs. Then it traumatizes them further with abusive training (food, sleep, and toilet deprivation + M&M conferences) they moral injury as they rise in the ranks. This is how we groom calloused leaders who fail to fix things or create emotionally safe environments. (If I had a dime for every "when I was coming up...") Add to the mix overwork and underpay, and the fact that this has all devolved into Stephen's observation of "professionals pitted against each other and silo paranoia (that) has increased" should surprise no one. TRUST: How long have we known physicians have the highest per capita #suicide, #overdose and divorce rates and done nothing? Decades. ?? #Healthcare systems in the US are largely designed to maximize patient volumes and corporate value (including at non-profits), but it didn’t happen overnight by one boogeyman/woman/binary executive’s design. (More like boogeysurance!) I'm increasingly convinced solutions need to include systemwide value-based payments based on #workforcewellbeing and retention metrics. Let’s show staff they matter by tying our financial future to their health. PSYCHOLOGICAL SAFETY: Physicians and nurses are also afraid to seek mental health care for fear of losing their jobs and licenses. We must get rid of credentialing that asks if we have a mental illness or SUD that could impact care, but not about diabetes, overweight, or cancers that could impact care. Instead let’s encourage universal therapy to normalize it! For all our docs, nurses, therapists, and other staff to heal and build back better, we need #traumainformedcare to address every one of these drivers by design. And do so including leadership, patients, payers, and policymakers in the process. No more singularly minded solutions or legislation. #mentalhealthishealth. We CAN and MUST do better in #healthcare! A serious look for a serious conversation as we kick off the week. Have you ever been in a situation like this? You’re a physician. A nurse suddenly snaps at you for what she perceives to be an oversight on your part. This nurse is competent, invested, and caring. You respect and value her work and feel honored to have her on your team. You deal with the perceived oversight and, later on in the day, ask to meet with her. No response. A couple of weeks go by, and you send out another invitation to meet. Again, no response. So, you schedule a meeting with her supervisor. You discover that there has been pent-up frustration, and are surprised that this has not been communicated to you directly? Supervisor: ‘Well, it’s not easy.’ Physician: ‘What’s not easy?’ Supervisor: ‘It’s not easy for a nurse to speak to a doctor about her frustrations.’ Physician: ‘It seems to me that communication is the issue. We wouldn’t have reached this point if there was direct and open communication.’ Supervisor: ‘This is not a nursing issue. It’s a medical issue.’ Friends and colleagues, this is why our healthcare system is failing. It’s not a nursing issue. Nor a medical, leadership, technological, financial, or staffing issue. It’s a problem solving issue. An ability to step back with a fresh perspective and ask ‘what went wrong and how can we make this better?’. Because solutions are only as effective as our ability to identify the real problem. When will we break down the silos and start speaking to each other to fix the problems that are plaguing our healthcare system? Problems that don’t require a degree in astrophysics, but simply common sense, good intentions, clarity, and creativity.
