Amplifying this excellent impactful work that I had a tiny bit of involvement in for which I’m very proud. Patient oriented discharge summary. You (or a loved one) is in the hospital. So many practitioners. So much information. When you leave the hospital it’s an impossibility to try and track all those people down if you have a question, or something after the rush of the hospital is unclear. This is a simple, but impactful and effective co-designed solution to solve the myriad of challenges a lack of access to information post discharge can pose. For you. In health systems, a ‘care pathway’ is a detailed care plan, charting each step for a specific patient population to make sure they safely and efficiently reach the best outcomes and quality of life. They are the foundation of our healthcare systems. They put clinical guidelines into action. The European Reference Networks (ERNs) are collaborating with national authorities under the new EU Joint Action for Integration (JARDIN) to develop ERN care pathways, that can be tailored and adopted locally in national healthcare systems.
