The rise of the administrative and anti-competitive third party adjacent business state has seen non-caregivers wielding power in the healthcare industry, overshadowing those whose primary focus is on providing care. Political operatives turn a blind eye as lobbyists representing large consolidated industry players dismantle the free market necessary for advancing care, embracing innovation, and ensuring affordable access. All this while billions in PAC dollars fund caregiver civil wars against eachother, as they fight for the crumbs that fall off the table, undermining access and stable working relationships required for patients in need of care. With the implementation of the Affordable Care Act (ACA), millions of healthcare providers were compelled to transition from being part of small independent groups to becoming employees under these conditions. This shift resulted in a loss of autonomy and influence, while adding significant overhead costs to sustain this employed model. Furthermore, when considering the government's consistent reduction of reimbursements from the Centers for Medicare & Medicaid Services (CMS) in a nation where this payer mix plays an ever expanding and pivotal role, the situation becomes even more dire. The sudden market correction in salaries following the COVID-19 pandemic, with caregivers now demanding higher wages, is akin to pouring kerosene on an already smoldering fire. It’s as unsustainable as our federal debt. In the United States, there is a lack of emphasis on preventative care and the availability of healthy, affordable food for the majority of Americans. It is disheartening to compare the cost and accessibility of nutritious food in other countries to the situation in the US, where unhealthy processed foods dominate the market. Subsidies for large-scale monoculture farming have led to the decline of small farmers, while promoting the consumption of over-processed foods that contribute to public health issues. As a society, we are increasingly promoting meatless agendas without addressing the root causes of unhealthy food production and consumption. It is crucial to reevaluate our priorities and policies to ensure a sustainable and health-focused approach to food production and distribution. Afterall, this industry is built on tertiary care, not preventative care. Ultimately, this fragile system is akin to a house of cards that is bound to collapse, affecting millions of individuals, yet sadly without any accountability. This is a stark reality of the political and administrative state - they often evade responsibility and consequences for the repercussions of their actions. For those who passively stand by and permit these injustices to unfold, they must understand that complicity through inaction is a form of consent. Silence can be interpreted as acceptance of the status quo.

Too many patients who would benefit from hospice move too late to fully benefit, or don't transition to hospice at all. Instead, they spend more time in home health than is advisable given their condition, resulting in insufficient care. Patients whose trajectory shows that they are approaching end-of-life care should be identified early to facilitate conversations with family and patients about hospice resources, compassionate care, and end-of-life planning. Hospice is a holistic care approach that encompasses all of a person's physical, psychological, social, spiritual, and practical needs. It's essential to recognize that hospice isn't just about dying peacefully, but also about living until the last moment. Hospice isn't "last breath care" - that misunderstanding must be addressed. As one quote beautifully summarizes, "You matter to the last moment of your life. We will do all that we can not only to help you die peacefully but also to live until you die."
