But did the target locations actually improve in the levels of workforce. This research is valid and indicates higher proportions or odds ratios of the pipeline program or special training program working. In Nebraska the FM choice of UNMC students was the desired result, an indication of success for the pipeline. This success worked for 10 - 12 times odds ratios in 70 target counties, but the levels stayed the same for decades We have to focus on the financial design for any hope for rural, CHC, inner city, or 2621 counties always behind. If less than 50 billion or less than 20% of primary care spending goes to primary care practices in 2621 counties with 40% of the population, it does not matter what training design is attempted. All will fail and have failed for 40 - 60 years in these counties. If the public plans remain incapable of carrying the costs of delivering care to Medicare and Medicaid patients, the deficits will remain. This is because these counties have weaker employers and weaker health insurance plans that cannot bail out public plans - as in counties with concentrations of workforce. You cannot fix with training what the financial design sets in concrete so stacked against care for 40% of Americans most behind. Failures for 50 years include 1. New types of graduates (NP PA FM) 2. Massive expansions of DO NP and PA 3. Special pipelines 4. Primary care schools 5. Student interest groups 6.Faculty mentors 7. FM Departments in every school, FM residencies in every state More revenue and less cost of delivery is required but the design says less revenue, less remaining in the budgets for delivery team members, more to do, and more costs of delivery added.

A few positive key points from a national community health center (CHC) study on research access and diversity. Of the 247 respondents, almost 82% said clinical trials are vital for medical innovation, 69% believe they benefit society and 75% believe adequate patient protections are in place. As part of a grant, a white paper and toolkit have been produced to support CHCs, research organizations, and life science companies in helping increase CHC involvement in clinical trials and studies of all types. The recent launch of the partnership-based center at Penn State Health in Hershey is a step forward in addressing health disparities across our communities. This new center brings together the expertise and resources of several organizations, including Penn State College of Medicine and Highmark Health, with a clear focus on tackling systemic health inequities. The goal? To understand and reduce the barriers to quality healthcare for all, particularly in underserved populations. By working together, these organizations are taking on critical research, community engagement, and educational initiatives aimed at closing the gaps in healthcare access and outcomes. Seeing collaborations like these gives me hope. When organizations align resources and expertise, they create an impact that goes beyond what any one group can achieve alone. This is the kind of community-based, action-oriented approach we need to build a healthier, more equitable future. Kudos to Penn State Health and its partners for their commitment to positive change! I?found?the?entire article by?Dr. Keith Norris and Dr. Arleen Brown, co-directors,?working to address inequities?through the UCLA Clinical and Translational Science Institute’s Community Engagement and Research Program to be informative. I found the following 2 points insightful: "It can be a steep, uphill climb. Cultural, social and socioeconomic factors continue to influence the lack of diversity in clinical trials. To recruit participants from underrepresented populations, you have to consider the reasons behind their lack of participation. Some of these reasons fall under the social determinants of health, such as lack of transportation and childcare. Other barriers are distrust of researchers, lack of knowledge about research processes and fear of potential negative effects or risk." "There is a moral and scientific urgency to increase diversity in clinical trials, build trust with communities and promote fairness. But it is our job to prove to our communities we are trustworthy. We must continue to address each trial and community barrier to optimize a representative group of participants. At the end of the day, all communities benefit from advancements in quality medical research."?
