26th - 29th March is PrEP Awareness Week. Join us by sharing this post and other posts this week. PrEP (Pre-exposure prophylaxis) is a medicine that can be taken by people at risk of HIV to prevent getting HIV. When taken as prescribed PrEP is highly effective and as part of combination prevention is one of the key interventions that will assist in achieving the aim of no new HIV transmissions by 2030. The latest data from the UK Health Security Agency (UKHSA) shows seven people are still being newly diagnosed with HIV every day in England. Although PrEP is highly effective in preventing HIV and available for free through sexual health clinics since 2020, there are currently significant inequalities in access and low uptake in women and ethnic minorities, due to a lack of information and awareness. Those who are aware of PrEP and making the effort to access it, are facing major barriers in navigating the healthcare system, with difficulties booking an appointment at sexual health clinics due to a low number of PrEP appointments, digital poverty, language barriers, and low understanding of the system. The racial inequities in access have been highlighted nationally by National AIDS Trust in their report “Not PrEPpared (2022) and it is crucial there are actions co-designed by underserved communities to tackle these inequities. #PrEPAwarenessWeek #PrEPEquity #YourSexualHealthMatters @gmpash

#DYK the 10-10-10 targets? The 10-10-10 targets focus on overcoming societal and legal obstacles to HIV services, aiming for significant reforms by 2025. These reforms are vital for creating environments where individuals affected by HIV can effectively access and use health-protective services. Currently, numerous countries face challenges such as HIV-related stigma, discrimination, gender inequalities, criminalization of certain groups and behaviors, and age-related consent barriers, all of which hinder health access and outcomes. The 2021 UN Political Declaration on HIV and AIDS, alongside the Global AIDS Strategy (2021–2026), sets forth ambitious goals requiring countries to make legal and policy changes. By 2025, the aim is for less than 10% of countries to have laws or policies that restrict HIV service access, for under 10% of those living with HIV and other key groups to face stigma and discrimination, and for less than 10% of women, girls, and key populations to suffer from gender inequality and violence.

What can state health departments do to improve legal and policy protections for people with HIV? All people deserve to live their lives free from stigma and structural discrimination, yet HIV criminalization continues to harm people with HIV across the United States, especially in communities of color. State laws and practices should be informed by science, but most HIV criminalization laws fail to meet this standard. Consequently, modernizing HIV-related laws is an essential part of the movement to end the HIV epidemic.
