In these pressured times, it feels more important than ever that we focus on what working together around place and communities can do to support better, more cost effective outcomes for the populations we serve. Glad that our work together with NHS Providers Julian Hartley Saffron Cordery continues to support this important conversation on making integrated care a reality. Newton Europe

It was a pleasure to write an article with Russell de Souza, ScD, RD and Fahad Razak. Thank you to Policy Options for taking an interest in this subject! https://lnkd.in/ezFWJH5m Canadians wish to age and die at home, but most die in hospital. As a result, the end-of-life is the most resource intense period of our lives. We need to move more resources toward community options and provide comprehensive support for patients and their caregivers. We will need a robust primary healthcare system to do this! And, in case you were wondering, here is the climate change argument for community-based approach to end-of-life care. https://lnkd.in/ggbs-kQ7
Russell de Souza, ScD, RD, associate professor in the Department of Health Research Methods, Evidence, and Impact, and Myles Sergeant, assistant clinical professor in the McMaster University Department of Family Medicine write “The end-of-life care, environment and health system ‘polycrisis’ needs a new approach.” Access the full article: https://lnkd.in/ezFWJH5m

“The four-part series, “The Invisible Shield,” shows how the field of public health has saved countless lives in the U.S., protecting people from the constant threat of disease, and helping increase lifespans. The documentary explores the hidden public health infrastructure that makes modern life possible. It also highlights the work of thousands of people—physicians, nurses, scientists, activists, reformers, engineers, and government officials—who have worked together over decades to improve health outcomes, from the days of cholera and smallpox through the COVID-19 pandemic.”

Kansas hospitals are an integral part of the health care system and are essential to vibrant communities across our state. While hospitals provide care 24 hours per day, every day of the year, they also are increasingly focused on improving the health of their communities beyond the hospitals’ walls. This issue brief summarizes results of a recent survey of Kansas hospitals to measure and describe population health efforts of hospitals. The Kansas Office of Primary Care and Rural Health at the Kansas Department of Health and Environment, Healthworks, the Kansas Hospital Association and the Kansas Health Institute partnered to create and conduct this survey of Kansas hospitals and health systems.
