It’s disheartening to see so many hospice companies falling short when it comes to the essentials of patient care and support satisfaction. Hospice care is meant to be a time of dignity and compassion for both patients and their families, but unfortunately, many organizations are missing the mark. One of the key issues is the lack of personalized care. Too often, care plans are generalized, with little attention to the unique needs of each patient. This leads to a decline in overall patient satisfaction and a disconnect in the very support systems families are relying on during an incredibly vulnerable time. Moreover, many hospice companies are hesitant—if not outright resistant—to adding additional services that could greatly benefit patients and families. Whether it’s expanding mental health support, offering alternative therapies, or integrating more holistic approaches, there’s often reluctance to innovate or invest in areas that could enhance the quality of care. Hospice care is a crucial support system for individuals with life-limiting illnesses, providing comfort and dignity during their final stages. However, according to the National Hospice and Palliative Care Organization (NHPCO), a significant portion of those who qualify for hospice services do not actually utilize them at the time of their passing? In fact, it's estimated that 47% of eligible individuals do not access hospice care when they could benefit from it the most. ?? This statistic prompts us to delve deeper into understanding why there might be underutilization of hospice services. Are there misconceptions surrounding hospice care? As a hospice advocate, I seize the opportunity to talk to community groups, individuals, and leaders about the hospice benefit whenever and wherever possible. Invariably, I hear stories about the "lead-up" to deciding to choose hospice for end-of-life care and all of the accompanying emotions involved. Many describe their experience as a roller coaster and the need to debrief about the events leading up to the decision to call us. It's an important ritual that must be respected. For some, it is a natural choice as they have experienced hospice care with a loved one or friend, and they see all of the benefits. For others, the referral comes as a complete shock, and we routinely hear that the family is in disbelief because no one has had a conversation with them about the seriousness of their illness or the fact that they are no longer responding to treatment. It can be a real gut punch. Many don't even know exactly why they are calling other than their healthcare provider told them to. At that point, the best we can do is to take the lead from the patient and their loved ones, actively listen, and help them navigate the maze of emotions they are confronted with. It's what we do in hospice. No two patients or families are the same, and no two care plans are the same. I'm proud of the work that the Intake Department at the hospice I lead does to meet people where they are. They listen with patience and empathy, advocate for you, and are passionate educators. They know full well that an hour-long conversation may lead to two hours or more and could involve multiple conversations with many family members or loved ones. It may involve multiple follow-ups - but they are fully invested in promoting patient choice and providing informed consent. The peace of mind that comes with making decisions before the eleventh hour is immeasurable - which is why I strongly recommend that you make your wishes known through advance care planning.
