But did the target locations actually improve in the levels of workforce. This research is valid and indicates higher proportions or odds ratios of the pipeline program or special training program working. In Nebraska the FM choice of UNMC students was the desired result, an indication of success for the pipeline. This success worked for 10 - 12 times odds ratios in 70 target counties, but the levels stayed the same for decades We have to focus on the financial design for any hope for rural, CHC, inner city, or 2621 counties always behind. If less than 50 billion or less than 20% of primary care spending goes to primary care practices in 2621 counties with 40% of the population, it does not matter what training design is attempted. All will fail and have failed for 40 - 60 years in these counties. If the public plans remain incapable of carrying the costs of delivering care to Medicare and Medicaid patients, the deficits will remain. This is because these counties have weaker employers and weaker health insurance plans that cannot bail out public plans - as in counties with concentrations of workforce. You cannot fix with training what the financial design sets in concrete so stacked against care for 40% of Americans most behind. Failures for 50 years include 1. New types of graduates (NP PA FM) 2. Massive expansions of DO NP and PA 3. Special pipelines 4. Primary care schools 5. Student interest groups 6.Faculty mentors 7. FM Departments in every school, FM residencies in every state More revenue and less cost of delivery is required but the design says less revenue, less remaining in the budgets for delivery team members, more to do, and more costs of delivery added.

4.5 years ago, I took a leap of faith and started my journey at Premera. It had been 12 years since I had been in the corporate world, as I focused on raising my amazing kids and being actively involved in my community. But when my youngest started kindergarten, I decided to test the waters with a 6-month contract. If I didn't enjoy it, I knew I could always walk away. I still remember my first phone interview with Paul Hollie at Premera. Something about that conversation made me realize that I either wanted to work with him or not go back to work at all. Luckily, after a series of interviews, I was offered the position of Social Impact and Rural Healthcare Support Specialist. To be honest, I had no idea what "Rural Healthcare" meant at the time. But I was passionate about community involvement and eager to dive into this new challenge. As I learned more, I discovered the unique healthcare needs of rural areas in Washington and Alaska. It became my mission to make a difference in these underserved communities. Now, after 4.5 years and more than $100M in grants, I find myself reflecting on the incredible programs I have had the privilege to partner with. From collaborative care models, rural nursing programs, and small equipment grants in rural and remote areas, to youth mental health programs, substance use disorders, and focusing on the intersection of behavioral health and homelessness, we are watching the results come in. Stories like this are the reason I love what I do. The work isn’t done yet; in fact, there’s still a long way to go to provide equitable access to healthcare to everyone in the community, and I’m so grateful to be part of the impact. #Premera #SocialImpact #RuralHealthcare #MakingADifference

I was invited by The Journal of Radiology Nursing to review and write a column on Dr. Peter Hotez MD PhD’s book, The Deadly Rise of Anti-Science. Dr. Hotez was a regular expert on MSNBC and CNBC during the pandemic. My review is a column in the October issue of the JRN journal. A snippet is below. “The book also calls for us to lean forward and assume our new roles with new armor to fight in public squares and to support others that are. Given the broader risk, there is space for everyone to play a role. There is no neutrality when one develops empirical maturity. The heavyweights in anti-science require counterweights of truth and empirical evidence. Our silence is complicity and it benefits no one but those that are on the side of foolishness, negative disruption and chaos. Civil liberties do not replace the common sense of empirical evidence. There is room for debate but ones that are sense-filled versus senseless arguments. The damage is also insidious with not only a growing number of dissenters but also a growing number of intelligent and well-intended folks and believers in sciences turning into doubters. This is a clarion call for activists versus ideologists in science and beyond to preserve what is right, just and empirically based and grounded in equity.” The Anti-Science movement impacts nurses and healthcare delivery overall. This editorial in AJPM Focus discusses the evolving identity of the Public Health Preventive Medicine (PHPM) specialty, highlighting its integration of public health and medicine: - Preventive medicine physicians frequently need to advocate for their specialty’s value, often aligning with contemporary movements like health systems management or lifestyle medicine. - Efforts to rename or rebrand the specialty have been proposed to enhance recognition and clarity. - A formal effort by AJPM Focus, involving a 4-year Delphi study with 90 experts, aimed to define the specialty’s focus areas. The study identified five key foci that form the basis of preventive medicine practice, guiding its teaching, research, practice, and policy. These Five Foci of Preventive Medicine are : 1. Community Medicine: Focuses on community-level interventions, including disease control programs and public education on health. 2. Population Medicine: Involves population-level interventions, integrating clinical care with public health strategies to address social determinants of health. 3. Clinical Preventive Medicine: Centers on prevention-focused interventions for individuals or groups, including lifestyle and integrative medicine. 4. Health Promotion and Protection: Involves true prevention strategies through social, environmental interventions, and policy advocacy. 5. Core Public Health Functions: Incorporates assessment, policy development, and assurance as foundational elements of PM practice. - To simplify and better define the specialty, the name “Public Health Preventive Medicine (PHPM)” is proposed. This aligns with public health foundations and distinguishes it from other specialties. - The preventive medicine specialty must continue efforts to define and communicate its unique identity and value within healthcare. https://lnkd.in/dFxeGAni
