A typical clinic visit for me on a Tuesday. Patient: my shoulder has been killing me. I was lifting something heavy in my garage and felt my shoulder "rip". I had immediate pain and can't lift my arm. I'm not getting any sleep. Me: Well, you are 55 and have significant RTC weakness. I'm pretty sure you have a rotator cuff tear (20 years in practice and performed over 25k shoulder evaluations). Patient: That's what I thought. I had the same thing a few years ago and it feels identical. Me: I'd love to get an MRI for you but insurance will deny it. We can try but they will make you do 6 weeks of PT first. Patient: PT costs me $40 co-pay each visit and I'm busy working and don't have time to take off to do something that isn't going to fix the problem. Me: I realize that and agree. We can try but I'm pretty sure we will get denied. This is BUCA 2024'. Prove me wrong. Is this how you want to be treated? No one does. Hang on. It's going to get better. We have had enough. The movement is coming.
