Twenty years after first seeing a knee replacement as a medical student, I've finally taken a leap and have performed a total knee replacement without needing to make any incision into the quadriceps tendon. Cutting into the quads tendon is standard for most conventional knee replacements and is what surgeons call a "medial parapatellar approach" to the knee. Instead, I've now done an "undervastus" approach (described in in the literature by a surgeon from Japan) where the whole vastus medialis muscle is carefully moved over the femur, without any disturbance of the quadriceps tendon. I was apprehensive but exposure was excellent. My US colleague Ryan “Saw-Bones” Molli, D.O., F.A.A.H.K.S., has nick named this the "combover" approach as the quads muscle is drawn over the femur rather incising the tendon. Ryan Molli, D.O., F.A.A.H.K.S., F.A.A.O.S. was very gracious to share some instructional videos with me before my first case with this technique. I'm very excited to see if this translates to a different recovery course for the patient. My main driver is to see if we can improve patella tracking in KA aligned knees. These are the common RTS stage tests done with force plates in #ACLREHAB with details on protocols and outcome measures. Keep in mind, this information is part of a very wide NET of data that assists practitioners in the decision to return an athlete to their sport. In my #ACLCOURSE (fully online), I cover, in Modules 8 and 9 other aspects such as the Cutting Movement Assessment Screen, Vestibular balance testing, Strength testing!!! (no one is talking about minimum test standards), Psychological Screens (TSK-11; RSI and IKDC) as well as the #controltochaos continuum of adequate exposures. Sure, we can use force plates to get data that is useful. But this alone is not the entire process to determine readiness and RTS. RSI is something we train, not something we simply test!!!!! I have found preoperative planning to be so important in my arthroplasty practice. To me, this case highlights the value in preoperative CT planning. While the glenoid is the usually the "straightforward" part of an RSA for fracture since there is usually no deformity, this patient had a very narrow glenoid vault (something that may be overlooked without preoperative planning). Here, I had some concerns about placing a boss in the narrow glenoid vault. Therefore, I did an off-label use of the Stryker Reunion baseplate with the Tornier USA Perform humeral fracture stem. Patient is doing well. 1. Do you get preoperative CT on all patient's undergoing total shoulder arthroplasty? What about those with fracture? Those with massive rotator cuff tearing seen on MRI? 2. Do you have any concerns about glenoid vault violation with a baseplate boss (if you use a system with a boss)? 3. Do you have any concerns about off-label mismatching? Will you match a different size glenosphere with polyethylene in certain cases (ie. revisions, etc...)?
