We do not waste your time matching patients with Medi-Cal to non-Medi-Cal facilities and vice versa. Nobody has time for that in the #emergencydepartment ! Fostering relationships with specialists, organizations, and people who refer patients to your health center is a great way to increase the number of new patients you see. Here are some excellent referral sources to whom you can start educating about your services, reciprocate the referral, and show your appreciation regularly. ?? Specialty providers: Like primary care providers, dental and behavioral health providers refer to specialists; specialists can also refer people to your providers. Run a report from your EHR to see who is referring patients to you and send them a token of your appreciation. Or, see who is not currently referring patients to you and ask to give their staff a presentation on the health center's services. ?? Church leaders: Church leaders often try to find healthcare services for their members. Bring them lunch and tell them about your services and how you serve people in their communities. ?? Human service organizations: Ask to be included on the staff meeting agenda at your local domestic violence shelter, public housing authority, job service, and transitional living facilities, and tell them about your services. Of course, stand behind your word anytime you promote your services to people who are referring you. If you promise they can be seen within a week, ensure that is true. CalAIM provides enhanced care management and community supports to address social needs. Enhanced Care Management: ? is a statewide MediCal Managed Care Plan benefit to support elevated comprehensive care management for members who have the most complex cases ? assigns members with complex needs a Care Manager to help them navigate their care and services across the physical, behavioral, and dental health delivery systems. We’re working with California Health Policy Strategies (CalHPS) to support a CalAIM Statewide Dementia Care Learning Collaborative to support community-based organizations that are serving older dual eligibles and persons with dementia at high risk of institutionalization to offer enhanced care management. Great investigative research by Bill Finerfrock (Movement is Life Board Member) on how methodology to determine medically underserved areas in America has not been updated in 30 (yes, thirty) years and why this matters! "If the federal government revised the HPSA/MUA/MUP methodologies to include the primary care workforce available in 2024, thousands of communities currently designated as underserved would see their shortage area classification removed." We absolutely have medically underserved areas that need support---we should have a process which accurately identifies them! After being in crisis, a peer support team member reached out to our EAP provider to connect me with a clinician. Two weeks later, and after some tears from the provider, I was told, “I can’t help you.” The term "culturally competent provider" is a buzzword in the first responder community. But what does it really mean? Evaluating someone as a “culturally competent provider” is too subjective, if you ask me. When I hear people outside the first responder community claim they have culturally competent providers, I see a ?? Given this subjectivity, first responders should be the ones vetting clinicians. So, I ask the following question: how does your department vet clinicians?
