Clinical nurse specialists can expand the capacity of health care systems, finding efficiencies and making room for the uninsured. ?? The setting: A level one trauma center in Detroit. ?? The problem: Inefficient processes meant that uninsured patients could not get follow-up care once they were released from the hospital. ?? The solution: Physicians would see the patient in the emergency room, make a diagnosis, and establish the game plan. From then on, Dr. Mitzi M Saunders and six other CNSs would take over their treatment. This meant that the physicians could focus on new patients who had a doctor. The procedural and cost efficiencies savings Mitzi found reduced five days of hospital treatment to four. On the fifth day, she and her team would hold a clinic for uninsured patients who needed care for progressive diagnoses of cancer or heart disease. ?? Why it worked: As a CNS, Mitzi was able to diagnose and prescribe. She was able to order the right tests and find low-cost medications that would work without side effects. ?? Outcome: Patients who would not have received follow-up treatment were given the care and medications they needed. ?? Cost savings: Her new process saved 4x the teams' salaries every quarter. What I Do Wednesday- Last week I had a client that went in for an outpatient procedure (the same one that had to be rescheduled due to a medication error). This client is a full care coordiation client. For local care coordination clients, we manage all healthcare. We make and attend appointments, we keep families upated, we make in-person visits, we coordinate care among all providers and faciities, we monitor hospital admissions, we facilitate discharge coordination from hospital, to rehab to home or facility and much more. We cover scoops to nuts when it comes to healthcare and resources. For this client he went in early in the morning for his procedure. We stayed with him until he went to surgery and followed care all the way through to his unexpected hospital admission. We kept the family and his assisted living facility updated every step of the way. Once hospitalized, we reviewed his chart daily, spoke with his bedside nurse at least once per day, visited him in person every day and coordinated discharge with the hospital case manager and facility director of nursing. On the day of discharge we were bedside, discussed discharge instructions, reviewed medications and durable medical equipment. Once back at the facility, we coordinated with staff to make sure everyone was on the same page and the care and handoff was seamless, including instructions on after care and pain medications. Family was fully informed from the moment he arrived at the hospital until he arrived and got settled back at his assisted living facility. Full care coordination is perfect for families that do not live close or are not able to manage healthcare for their loved ones. Although this client's procedure did not go as planned, he received excellent care and follow up. "North Carolina, like many states across the nation, is struggling with doctor shortages. Far too many residents have difficulty finding reasonable access to a primary care physician, which can lead to poorer health outcomes, especially in rural areas like Appalachia in Western North Carolina. Fortunately, North Carolina legislators could take a major step in addressing the problem with a simple fix. It’s one that 28 other states have already implemented. A?new report?released by the Knee Regulatory Research Center at West Virginia University finds that granting trained nurse practitioners full scope of practice will alleviate provider shortages and improve health outcomes for more North Carolinians." A moving article about the current physician shortage in New Hampshire, especially within primary care. Similar situations are occurring across the country. "Medical practices are full, not taking new patients or putting everyone on a waitlist. Health care systems and outpatient practices are using more nurse practitioners and physician assistants than ever to diagnose and treat a wider variety of conditions, reserving the few doctors on staff for the most complex or serious ailments." Enhancing Early Intervention for Dizziness & Balance Issues: Bridging the Gap with Vestibular Training At DizzyCare Clinic Academy, our vision is for patients experiencing vestibular and balance issues to receive an early diagnosis and timely vestibular rehabilitation.
