I'm so excited to see this article in my inbox this morning. The supply and demand imbalance of licensed clinicians to people in need needs new solutions for care delivery. Highly trained lay helpers like Coaches are a huge lever to increasing access to care. Seeing giants in the field acknowledge this, is true progress. There is a fallacy that having a license somehow means that you will provide good care. It's simply not true. Ensuring high-quality care from ANY provider comes from: ?? High-quality training in evidence informed treatments, cultural humility and empathy ?? Rigorous measurement to ensure outcomes ?? Training a diverse set of providers to meet the needs of those in need ?? Work environments that treat our providers the way we want our patients to be treated (including paying a wage commensurate with the work, time for clinical support and documentation) So exciting to see articles like this. People tell me that they find it really hard to navigate the system and get help on the NHS. There are some really fantastic NHS services for ARFID but they are clearly stretched. I am a fierce lobbyist for access to support on the NHS whenever this is appropriate. One service I often provide is my Triple A programme ie Assessment, Advice and Advocacy. By conducting a thorough assessment, highlighting physical and mental health risks and where relevant the role of neurodiversity, I am often able to help people access further support via the NHS. This might be specialist NHS dietetic support, psychiatric assessment, specialist speech and language therapy or psychological therapy such as CBT for ARFID or graded exposure therapy. Other times people meet the criteria for multidisciplinary support in a specialist centre. I am always happy to provide interim dietetic support for people on waiting lists and will work collaboratively with my NHS colleagues providing reports as required. I also use my network to put a team around my patients privately where that's necessary. Sometimes people are able to make such good progress in the interim that when they reach the top of the waiting list they no longer meet the referral criteria for specialist centres. It's very rewarding to support people to find hope and better health, so this is great. But of course not everyone can afford private healthcare. It is my hope that this Eating Disorders Awareness Week will encourage more funding for stretched NHS services and improved awareness across the private health insurance network, so that everyone can access great support.
