2 on 2 : Two of our interesting images have been published in the last two issues of CNM. Imaging of lymph nodes with FAPI is an absolute dilemma. In our case, FAPI was significantly superior to FDG. It seems that dual PET imaging may improve accuracy for staging in gastrointestinal malignancies. BONUS: In the latest volume, a case of triple expressor DLBCL mimicking other malignancies without lymph node involvement was presented. 55 YO male with a large oropharyngeal partial necrotic lesion. Diagnosis: Follicular Dendritic Cell Sarcoma FDCS is rare in extra nodal sites, but tonsillar involvement is known. Here, in the first look it resembled SCC, but something was off about the morphology. So please keep in mind this diagnosis when u see an unusual looking neoplasm in the tonsil. Initial panel of IHCs involved pan CK, EMA, p63, desmin, myogenin and S100. When all these came negative, CD21 and CD23 were done which came out positive. Learning points: - Painless solid mass in nodal and extranodal sites which behaves like low grade sarcoma - Oval to spindled cells, eosinophilic cytoplasm with syncytial borders arranged in fascicles, whorls or storiform patterns - Binucleate or multinucleate forms - Nuclear pseudoinclusions - Perivascular lymphocyte cuffs
