Let’s get clear on what’s really happening: the Bates aren’t “advocates”—they’re mercenary ambassadors, paid to spotlight NxStage as if it’s the one and only option for patients seeking independence through HHD. This approach falls dangerously short of genuine advocacy Real advocacy is transparent, balanced, and patient-centric. It’s about empowering individuals to make informed choices across all available treatment options, not just the one backed by a sponsor’s dollar. But what NxStage Medical, Inc. is doing with its promotional campaign and carefully curated social media moments is far from empowering—it’s straight-up brand ambassadorship masquerading as “freedom.” And it’s glaringly one-sided. True advocates would discuss other #HHD solutions, like Outset Medical, Inc. #Tablo, which offers more flexibility for some patients in a very different way. The message here is superficial at best and misleading at worst. It sidesteps the real obstacles of HHD: intense training, complicated setup, constant monitoring, and a lifestyle that revolves around safety measures. The day-to-day routine of home dialysis is anything but “freedom” in the simplistic sense that NxStage’s ads imply. For most patients, HHD means careful adherence to medical procedures, a continual focus on sanitation, and no small amount of physical and emotional labor. And to all the #FreseniusMedicalCare employees endorsing this as advocacy—really? How can paid sponsorship, with its polished, choreographed messages, be anything close to real advocacy? Real advocates champion patient rights, push for better access, and inform people about ALL available options, not just the one they’re paid to promote. If you genuinely believe in patient empowerment, why not take this so-called advocacy further? Why not ride those motorcycles to Washington or present this “freedom” pitch to groups actually working for patient rights, like WaitlistZero.org? This entire campaign by NxStage, with the Bates front and center, is a grotesque display of paid sponsorship. And that “freedom” slogan? It’s a shiny but hollow tagline. Where’s the talk about the everyday realities of HHD? Where are the resources for those navigating insurance coverage for multi-state listings, the logistical challenges of portable dialysis options, or the realities of maintaining health in a home dialysis environment? Patients deserve better than a one-note brand ambassador campaign that looks more like an ad for a high-end lifestyle product than a real option for chronic illness management. NxStage is shamelessly pushing a misleading narrative, playing a cheap trick with the idea of “freedom” by showing a couple joyriding on mopeds or tricycles, pretending that a life on HHD is just a carefree tour across America. What they’re selling is a fantasy, not reality, and this ad misses the mark in every possible way. Let’s get real about what it takes to live on HHD. It’s not “snap and tap” like the video suggests; it’s grueling, constant monitoring, strict sanitation, exhausting cognitive strain, and relentless pain. Where are those realities in this clip? Instead, we see a couple bouncing around to their favorite tunes, acting like setting up dialysis is a breezy two-step routine. Advocacy is about raising awareness, advocating for legislative change, and educating patients—not dancing to a beat on a TikTok clip. When was the last time these “advocates” rode their mopeds to the front steps of US Congress? Or wrote a letter to the Senate, met with U.S. Department of Health and Human Services (HHS), pushed legislation through US Ways and Means? My guess? Not recently—they’re too busy playing around on social media, delivering an over-simplified message to vulnerable patients. If this is what NxStage calls “advocacy,” then we’re in big trouble. True advocates don’t gloss over the gritty reality of HHD. They’re out there—working behind the scenes, talking to Congress, challenging Centers for Medicare & Medicaid Services, and advocating through organizations like Waitlist Zero. NxStage, by contrast, is flooding feeds with a grotesque display of paid ambassadorship, trivializing the demands of HHD for a cheap “feel-good” story that should be removed immediately.
