As the article suggests, hospitals are not reimbursed nearly enough for their out of pocket costs to deliver behavioral health services, but as the article also wisely notes, outpatient behavioral health services actually fair even worse in terms of reimbursement. Providers being reimbursed $0.32 cents on every dollar it costs to deliver care is unsustainable. For outpatient providers, the federal Certified Community Behavioral Health Clinic (CCBHC) designation offers hope and is a step in the right direction. Although the reporting requirements are significant, the care model is more holistic and the approach to reimbursement incentivizes quality and visit efficiency while determining payment rates using actual costs. We have a long road ahead of us, but at least the federal government recognizes this inequity. Some states (New York, to name one) are also taking matters into their own hands, and are working on improving payor reimbursement for safety-net providers.

The 2021 American Rescue Plan Act allowed states to receive federal matching funds to extend pregnancy-related Medicaid eligibility to twelve months postpartum. In Health Affairs’ April Perinatal Mental Health & Well-Being issue, Sarah Gordon of Boston University and her team of coauthors examine associations between postpartum Medicaid expansion in Colorado and mental health care utilization among low-income people with Medicaid-paid births. The researchers determine that retaining postpartum coverage was significantly associated with increased use of prescription medication or outpatient mental health treatment. They find that “remaining in Medicaid was associated with a 20.5-percentage point increase in the rate of having at least one outpatient mental health visit or filling at least one prescription medication for anxiety or depression in the postpartum year.” Read the full paper: https://bit.ly/4arWqGT

Your employees are parents and have elderly parents who depend on them—therefore they are “caregivers.” CDC research shows caregivers have a 25.6% prevalence of lifetime depression compared to 18.6% for non-caregivers. Smoking rates are also higher among caregivers at 16.6% versus 11.7% for non-caregivers. As the CDC report states, "Strategies for supporting caregivers are available, and integrating these with existing programs to address mental health and chronic diseases among this population might improve caregiver well-being." The ongoing hospital crisis in Ontario, exacerbated by staff shortages and delays in care due to the COVID-19 pandemic, has highlighted the urgent need to address the challenges faced by Alternate Level of Care (ALC) patients. These individuals no longer require hospital-level care but remain hospitalized due to a lack of suitable community settings with necessary supports for discharge. Among the most vulnerable to this issue are individuals with developmental disabilities who also have a psychiatric condition, often referred to as a dual diagnosis. This group faces a significantly higher risk of becoming ALC patients and, in some instances, endure extended hospital stays, which can lead to adverse outcomes for them and treatment delays for other patients in need of hospital care. CAMH’s H-CARDD team conducted a cross-sectoral project to identify the fundamental principles and core components necessary for the successful transition of adults with a dual diagnosis who are currently ALC patients in hospitals, into community settings.
