Earlier this year, we (Wohl E et al. A&A Practice) showed that evaluating anesthesia practitioners based on ICU admission after ambulatory surgery is invalid. Can automate evaluations but misleading criterion. New study considers all anesthetics for surgery and postoperative length of stay. It also shows that it is invalid to consider LOS as associated with anesthesiologists' and nurse anesthetists' quality of clinical care. https://lnkd.in/gW262ryJ

Your anesthesiologist might make a massive difference in your life before you ever meet them…by making sure you DON’T meet them, to undergo the planned (elective) anesthetic, until you are fully optimized. When I review my patients’ charts preoperatively, I begin by reading the surgeon’s H&P plus the medical clearance note, if one is available. Anesthesiologists & surgeons could assume that if an internal or family medicine physician has cleared a patient for surgery, then the patient is ready for surgery. Seems simple and fair. But I don’t follow it. Even after reading that the patient is “cleared,” I scan the rest of the chart, including Care Everywhere, to see if any other doctors have recently evaluated the patient and for what. It might reveal that the patient developed a bad URI and went to an urgent care center a few days after the internist’s clearance. Or I might, as I once did, find a note from an ophthalmologist that the patient was being treated for optic disc edema. The next ophthalmology follow-up was scheduled after surgery. I knew, as an anesthesiologist, that this surgery would include steep Trendelenburg positioning. So I contacted the ophthalmologist who, upon hearing about surgery, required a follow-up exam before any clearance. The result? Optic disc edema worsened, (elective) case postponed until further treatment, serious visual consequences potentially avoided. Everywhere, anesthesiologists are using not only their knowledge of anesthesia, but also the knowledge gained about other medical specialties during M.D. (or D.O.) school, to work silently and diligently behind the scenes to optimize patient safety and quality preoperatively.
