We must invest in primary care at a level that reflects its immense value. Primary care is the only part of the healthcare delivery system where an increased supply is tied to better population health and equitable outcomes. Yet, as the Milbank Memorial Fund has documented, the United States continues to underinvest in primary care. Currently, national spending on primary care is only 5-7 cents of each healthcare dollar. In an April 15, 2024, Opinion in Healthcare Dive, Christopher Koller, president of the Milbank Memorial Fund; Kyu Rhee, president and CEO of the National Association of Community Health Centers; and R. Shawn Martin, executive vice president and CEO of the American Academy of Family Physicians explain that chronic disinvestment and inadequate training have created a shortage of primary care workers. They call for higher reimbursement for primary care and using more of the public dollars going to clinician training for creating a stronger primary care workforce. Koller and colleagues also recommend expanding the footprint of community health centers, which contribute to improved health outcomes in underserved communities. “It’s time to strengthen our fragile primary care system to ensure it delivers the comprehensive, affordable care Americans so desperately need,” the authors say. “Access to high-quality primary care for everyone should not be an aspiration, but an expectation that we – as a nation – have an urgent duty to fulfill.

Excellent op-ed in Modern Healthcare by AHA CEO Rick Pollack and Catholic Health Association CEO Sister Mary Haddad. They discuss the essential contributions nonprofit hospitals make to their communities?and?how essential financial stability is for nonprofit health systems to achieve this work. The learned authors state “Barely making ends meet or operating at a loss should never be the goal or an acceptable status quo for any nonprofit healthcare organization”, elaborating that bad economics impede health systems abilities to deliver services and improve outcomes. We often post here about healthcare inequity - the postcode lottery; the rural healthcare worker shortage; the layers of difficulty communities in the bush experience accessing healthcare services..... But the greatest healthcare inequity is that experienced by our first Australians living regionally, rurally and remotely. On National Closing the Gap Day we acknowledge the ongoing inequality in healthcare access for our first Australians. With almost 60% of Aboriginal and Torres Strait Islander Australians residing in the most rural or remote areas of our country, there is much work to do in this space.

In light of Lord Darzi's recent report on the NHS, it's crucial to reassess how community and primary care are structured. This report presents a significant opportunity to discuss whether community trusts should be reorganised, with substantial parts of their portfolio managed by General Practice. Many of these areas are under block contracts, that commissioners seem unable to break up, and that are very difficult to show value for money.? GPs have consistently demonstrated their deep understanding of their patients and communities, enabling them to swiftly adapt resources and services to meet patient needs. Unlike other healthcare sectors, General Practice has a track record of not operating at a deficit, highlighting its efficiency and effectiveness in resource management. This moment calls for a constructive dialogue on leveraging the strengths of General Practice to enhance community care, ensuring that services are both patient-centered and economically sustainable.
