The U.S. Food and Drug Administration has announced a new initiative, Home as a Health Care Hub, to reimagine the home environment as an integral part of the health care system. The goal is to advance health equity for all people in the U.S., including millions who have no or limited access to health care systems. By putting people, not systems, at the center of care, the initiative allows the health care system to prioritize resources for those with critical needs and tailor care for those managing chronic conditions. Dr. Philpott definitely has the right answer for more effective and just plain more access to primary care in Ontario. I am confident her report will be well researched, well-rounded (considering the implications and benefits beyond primary care access in terms of health system impacts), and practical. An excellent report, however, will not be enough. It would be wonderful if practical, implementation advice to government could be included - I am thinking particularly of the need to address the challenges of our adversarial party system and the sometimes short time horizon of a government in power vs the time needed for full implementation of a new primary care model.

As so many new Digital Front Doors are flooding our healthcare system the question is can we have a No Wrong Door Strategy? As access to free healthcare becomes harder , how do we make it easier for consumers to navigate the health system to get the help they need. In the absence of technology that supports clinical handovers and easy digital patient journeys and a good portion of the cohort with low digital literacy or even digital poverty ,do we need a human solution as a stepping stone to help consumers navigate an increasingly complex health system? Healthcare quality improvement efforts tend to focus on micro interventions, and have low yield at impacting healthcare quality at larger scales. Accreditations and volume based payment for services, without systemic reforms barely shift user experience and population health outcomes in a positive direction.
