The European Association of Cardiothoracic Surgery (EACTS) and the Society of Thoracic Surgeons (STS) have classified the aorta as an independent human organ in their guidelines on aortic surgical treatments. “The recognition as an organ puts the aorta on a par with the heart, lungs or brain. This is a big step,” said Prof. Martin Czerny, Chairman of Cardiovascular Surgery at the University Heart Center at Freiburg University Hospital and coordinator of the new guidelines. These were published in “The Annals of Thoracic Surgery” under the title “EACTS/STS Guidelines for Diagnosing and Treating Acute and Chronic Syndromes of the Aortic Organ” (—> https://lnkd.in/gcNFpTkU). They describe the procedure for diagnosing and treating diseases of the aorta such as aortic aneurysm. To date, it has been common practice for diseases of the aorta to be treated either in cardiac surgery or in vascular surgery, depending on the type and location. The new classification of the aorta as an organ has far-reaching consequences, from the division of medical departments to the specific care of patients. The new guidelines clearly recommend bundling the care of the aorta in a separate specialist area, of course in close coordination with other specialist areas. The guidelines could therefore also have an influence on the curriculum of medical schools and specialist training.

One recent trend that has interested me while working as a critical care nurse has been the incidence of post operative atrial fibrillation following cardiothoracic surgery. Statistically, our patients have a 25% likelihood of having a post operative atrial fibrillation (POAF) event. This is typically contributed to pericardial inflammation due to oxidative stress and ectopic misfiring as well as often some sort of electrolyte imbalance exists as well. Studies have shown that preoperative administration of a low dose b-blocker to cardiac patients has shown a 48% reduction in risk of POAF. This an ultimately improve patient outcomes as post operative atrial fibrillation is often associated with stroke, thrombophlebitis, MIs and extended hospital stays. This is an area that I believe needs to be researched more extensively so that better treatment guidelines can be made regarding the use of beta blockers in reducing cardiac arrythmia events in patients that are known to be high risk. Can intraoperative driving-pressure-guided ventilation reduce the PPC risks? ???? ??A meta-analysis of 13 randomised controlled trials (RCTs) investigated whether driving pressure-guided ventilation can reduce the risks of postoperative pulmonary complications (PPCs) after cardiovascular and non-cardiac surgery. ??The analysis included 3,401 adult patients who underwent both types of surgery and received either conventional or driving pressure-guided ventilation. ?? The findings indicated that driving pressure-guided ventilation was associated with a lower risk of PPCs (RR 0.70, 95% CI 0.56–0.87, P=0.001), as confirmed by trials sequential analysis. ?? Furthermore, subgroup analysis showed that the association was mainly observed in non-cardiothoracic surgery, suggesting conclusive results and a lower risk of pneumonia (P< 0.0001). However, this association was not observed in cardiothoracic surgery. ??
