Did you know that: ·???????Diabetes-related cardiovascular mortality is 66% higher in the most socially vulnerable areas, with nearly 40,000 excess deaths every year. ·???????Diabetes disproportionately impacts racial and ethnic minorities, and low-income adult populations. This November is National Diabetes Month, which aligns with HEO’s mission to promote health equity, particularly for underserved communities facing significant challenges related to social determinants of health.
