Is it time for your aviation medical? All pilots and Air Traffic Controllers see a Designated Aviation Medical Examiner (DAME) for their scheduled medical. A DAME has advanced training in aviation medicine and is a delegate of the Civil Aviation Safety Authority (CASA). Preparing for your aviation medical can be a stressful time of year. How many pilots turn up with a sky high blood pressure? It’s not the coffee you had this morning, it’s not because you rushed. I get it, sitting in front of your DAME can make or break your career. But sometimes an elevated Blood pressure can be a warning sign of your heart and kidney function. If you genuinely don’t have hypertension, keeping a diary of your home BP readings for a week prior to your medical can help show the DAME the high reading on the day of your medical isn’t consistent. We worry about a blood pressure of more than 150/90. This will trigger us to order additional tests and find out if your heart or kidneys are under strain. This of course can be a warning sign your heart isn’t as fab as you thought. CASA also approve most medications for high blood pressure that your GP can prescribe. If you are over 40, your medical will include an onsite ECG, we plug in your data to a cardiovascular risk calculator to see if you need an exercise stress test on the treadmill with the cardiologist externally. This is more common when you are over 50. See your GP at least twice a year for a check up, including a skin check and other screening such as a bowel poo test. Otherwise happy flying and yes sit back relax and dream of a tropical holiday while I take your blood pressure for your medical.

"Bill" once had an EMS medical director that required agency paramedics to "pass" his scenario-based evaluation before being cleared to function as a paramedic within the organization ... which sounds fair and we can all likely understand the premise behind this ... but his (MD's) scenarios were so multi-faceted, unrealistic, and didn't follow any traditional rules of established patient triage practices (particularly during his MCI scenario), that it caught many testers off guard. "Bill" was recently hired from outside of the agency to fill a high-ranking position. After completing (and "passing") this potentially job-ending test, "Bill" suggested to the chief (his direct boss) that he fire the MD and bring in someone new ... someone who didn't promote such annecdotal and unrealistic evaluation methods. "Bill" wasn't granted that request. To "Bill's" learning afterwards, the department maintained a roster where approximately 25% of its EMT employees were actually state-licensed paramedics who simply chose not to function as paramedics in this agency because of this MD's evaluation process (among a few other reasons). Talk about something being wrong within an organization! Testing, evaluating, and anything that may be job-impacting needs to be vetted, objective, and realistic. If your agency/department doesn't follow this approach, you're only destined for failure (think about promotional exams/processes, etc.). This agency experiences a >20% turnover rate each year (with over 120 employees staffing the agency) ... and they wonder why ....

If there is a medical emergency onboard a cruise ship then it's not just the medical staff that needs to be properly prepared. The ship also has a stretcher party who help with moving the casualty. But is it enough that they just know how to correctly use a stretcher and use correct manual handling techniques? No, it is not. They should be trained to assist the medical staff because in a genuine emergency it gives you so many more sets of hands to help out. Things like: ?? Regular BLS scenarios means the medical staff can be freed up to focus on the ALS interventions. ??Setting up the oxygen as per the medical staffs needs. BVM? no problem. Non-rebreather? easy. Need the Pulse Ox? just ask, they should know what it looks like. ??Knowing what's in the grab bag. Being able to identify a piece of equipment can significantly reduce the amount of time taken to find it when asked to pass it because the Medical staff have their hands on and can't get it themselves. ??Good communication. Learning how to work as a team is all about a slick Comms plan. Regular training can imbed great communications and that will directly impact the outcome.
