“Addressing the Growing Need: Importance of Geriatric Medicine in Bangladesh” Geriatric medicine plays a crucial role in Bangladesh due to its rapidly aging population. As life expectancy increases and medical advancements allow people to live longer, there’s a growing need for specialized care tailored to the unique health challenges of older adults. Geriatric medicine addresses issues such as chronic diseases, mobility issues, cognitive decline, and polypharmacy, helping improve the quality of life for seniors. Additionally, promoting healthy aging and preventive care can alleviate the burden on the healthcare system by reducing hospitalizations and long-term care needs. Investing in geriatric medicine in Bangladesh is essential for ensuring comprehensive healthcare services that meet the needs of an aging population.

Pleased to share with you a video-discussion about the importance of American Board of Internal Medicine certification in Geriatric Medicine. Undoubtedly, our population is aging exponentially and many older adults travel with multiple comorbidities. Having expert geriatricians-- fellowship-trained and board certified-- is critically important. Not only does board certification reassure the physician that he/she is at a level of expertise that is evidence based, timely, and acutely relevant to the delivery of high level patient care, but it also reassures patients, families, and caregivers that they are receiving comprehensive value based care. Certification moves us beyond "just caring" for older adults towards a commitment to the care of older adults. To be sure, physicians who care for older adults in these ageist times where relative value units (RVUs) and complex medical billing can impact choice of patients and time spent in care, are admirable. However, doing so does not make these physicians geriatricians. Being fellowship trained, with objective testing and vetting of that knowldge through board certification signifies an authentic geriatrician, verifies the geriatrician's body of knowledge, and underscores the geriatrician's preparedness to handle the cognitive, physical, physiological, and psychosocial complexities of aging. Remember, geriatricians do not treat disease; geriatricians treat people with disease. ?? Personalized medicine is a must nowadays.? In Geriatrics, we need to be especially careful with drug interactions and side effects. When treating hypertension, we should consider the following targets: Clinic Blood pressure ABPM / HBPM ???? Age < 80 years 140/90 mmHg 135/85 mmHg ?? Age > 80 years 150/90 mmHg 145/85 mmHg Based on NICE guidelines. If we don't consider these targets in the elderly population, our patients can face the most common symptoms of hypotension like tiredness, confusion, sleepiness, and/or delirium. Prevention: - We can make significant impact by understanding our patients as a whole, not just their pathologies.? - We can conduct accurate clinical history. - We can consult the BNF for safe prescribing.? - We can always offer lifestyle advice and consider the patient’s ideas, concerns, and/or expectations regarding their conditions, management, and outcomes.
