"If you didn't chart it - you didn't do it" is a common motto for nurses. Why? Documentation ensures continuity of care, legal compliance, and patient safety. When a cyberattack happens and you can no longer chart electronically, you don't stop charting.... you move to paper. Most people don't even calculate a tip without their phone anymore and you're asking healthcare professionals to do high pressure math, in real-time, while emergencies are circling their workspace. Paper charting leaves more room for error with doses because doctors aren't getting the most accurate log of every input and output to decide the best course for a patient's treatment plan.
