Illinois’ new Medicaid demonstration program that supports facilities that provide comprehensive, around-the-clock behavioral healthcare and substance abuse treatment is set to begin in October. [Health News Illinois] Kristine Herman, chief of the behavioral health bureau at the Department of Healthcare and Family Services, told members of the Medicaid Advisory Committee that they are working on a “robust communication and technical assistance strategy” to ensure all participating clinics can begin offering the necessary services by Oct. 1. [Health News Illinois] The initial?19 mental health and substance use treatment providers?in the demonstration program must provide care coordination, 24/7 access to crisis services and serve all patients, regardless of their ability to pay. “These agencies will serve anyone who comes through their doors, regardless of the individual's insurance status, and they're going to ensure that these individuals have access to a very broad array of mental health, substance use, recovery, and physical health screening services, as well as referrals and coordination with any other services that the individual may need,” Herman said. An article recently published in the LA Times reported that Blue Cross and Blue Shield of Georgia informed its members that they will not pay the claim for individuals who are treated in the Emergency Room (ER) for a problem later deemed to have not been an emergency. Really? The notice used clarifying language like “prudent layperson” and “average knowledge of medicine and health”, but who makes that determination? Should it be made by insurance company executives? ????***Sucess Story*** ???? Nothing like starting out the new year with a great win. How does Walk On Clinic measure success? Not everything has to do with $$$ or does it? Please meet “Rosie”; Wink wink we don’t treat and tell but I have her permission to share. Rosie came to our clinic because she was not feeling well. I walk a very fine line every day, and recent posts about the tragic assassination of a CEO of a major health plan have been a stark reminder of that balance. The majority of my career has been spent either working for a health plan or closely with them. Yet, in my personal life, I’ve witnessed firsthand the barriers to care that these organizations can create. Fortunately, my insider knowledge proved invaluable during my daughter’s journey navigating the healthcare system while facing a chronic, undiagnosed rare disease. ?? I leveraged what I knew about health plans—things that most people understandably don’t know or even have access to. For example, when her out-of-network care was initially denied, I didn’t just resubmit the appeal. Instead, I relied on my knowledge of the standards health plans must meet for specialist access (# of miles to a persons home for a particular specialist) and the clinical guidelines they follow. I did my research and demonstrated that the specific care she needed by a very specialized surgeon wasn’t available within a certain mile radius of our home, which forced them to approve the out-of-network care. I also understood the role of case managers—how they can advocate for patients and even grant one-time exceptions to work outside the rigid “black-and-white” guidelines. Health plans employ many compassionate nurses, physicians, and other healthcare professionals who want to help, but they’re often bound by clinical guidelines, yet we know that approach often oversimplifies the complexity of medical conditions. Still, it shouldn’t take a mom who’s a nurse with a career in health plans to navigate the system. Accessing necessary care shouldn’t be this complicated for anyone. The process must become simpler and more transparent so that every family has a fair chance, regardless of their background or expertise. I don’t know the answer. I certainly am grateful for the experience we had and not going into tens of thousands of dollars worth of medical debt. I do know that when I saw a recent post showing the CEO salaries of the largest #healthplans today I felt ill.
