Postinfarction ventricular septal rupture (PI-VSR) often presents as cardiogenic shock with newly formed left-to-right communication. In such cases, mechanical circulatory support can provide hemodynamic stabilization during the perioperative period. We report a case where an undiagnosed ventricular septal defect (VSD) was found during PI-VSR closure with a percutaneous circulatory support (pCS) device. Sharp recanalization of a CIA occlusion by use of a 4 F BeBack catheter This is a 64 y-o female patient with a short occlusion of the proximal right CIA with stage II b disease. Retrograde puncture and cannulation failed to enter into the true aortic lumen and the guidewire travelled in the subintimal aortic space. Antegrade cannulation from a cross over approach using a 4 F Sim 1 catheter failed despite use of different types of gw mainly due to a very rigid calcified plaque. Leaving the Sim 1 catheter as a landing point, a 4 F Beback systm was advanced in a retrograde fashion and the needle was targeted towards the catheter tip. When the wire entered the aortic lumen, a 5 F catheter was advanced and the gw exchanged for a 0.035 in gw. The occlusion was primarily stented by use of a balloon-expandable stent graft for safety reasons.

TES is an adjunctive technique that may optimize sealing zones and luminal aortic diameter during endovascular repair of subacute and chronic post-dissection. Although no arterial disruptions or target vessel loss occurred, but patients with acute dissections are prone to technical failures related to dislodgement of the lamella.

After more than 200 Carotid Artery Stenting (CAS) procedures with common carotid endoclamping and flow inversion cerebral protection we added a new device to our tool-box. The Walrus balloon catheter by Q'Apel Medical was fast and simple to prepare, offered excellent navigation and stability throughout the procedure. CASE STUDY / FULL WOUND CLOSURE OF STAGE IV SACRAL PRESSURE INJURY Using NIRS imaging to confirm the improvement of StO2 allowed efficient wound care to completely heal the stage IV pressure injury in 35 weeks, faster than the national average healing time for this type of injury.
