While I working with a resident (physician-in-training) in clinic, he mentioned he wanted to improve his efficiency doing in-office skin procedures. However, he didn’t want to prep the patient’s chart prior to meeting them because some of his actions would feel wasted. He preferred meeting the person first, deciding exactly what supplies were needed, and then ordering these supplies. He expressed frustration with how long it took from the time the patient was roomed to starting the procedure. He didn’t recognize these constraints: certain supplies for safety reasons could not be stored closeby, these supplies required an electronic order to confirm accuracy, he’d have to leave the room to order them, and he had no EHR ordering shortcuts made. We discussed how his goals (productivity) were at odds with his instinctive behaviors (efficiency). While I also disagreed with some of our clinic’s enforced constraints, we instead focused at that time on how to reshape his flow to minimize the negative impact on his productivity. My take: This resident was seeking more productivity while ironically focusing on efficiency. These aren’t mutually exclusive, and each has it’s place depending on our daily work needs: ·?Clinical productivity – seeing the patient and completing the work in the quickest amount of time. Think of sprinting. This mode costs more in mental energy but costs less in time. This would look like creating ordering shortcuts, ordering all possible supplies before going into the room, and accepting that any unused supplies would have to be restocked later. ·?Clinical efficiency – seeing the patient and spending the least amount of effort. Think of a light jog/walk. This mode costs less in mental energy but costs more in time.
