World Suicide Prevention Day 2024 calls for a deep exploration of the structural determinants and root causes of suicide, as these issues often go unaddressed in mainstream prevention efforts.?? Suicide is not simply an individual or psychological problem; it is a social and structural issue deeply tied to inequality, marginalization, and systemic oppression.???? Structural determinants such as poverty, racism, ableism, and heteronormativity create environments where marginalized individuals experience heightened vulnerability to mental distress and suicidal thoughts. ?? People facing systemic discrimination—whether based on race, gender, sexual orientation, disability, or class—are often denied access to supportive resources, affordable healthcare, and community-based care, which could otherwise mitigate feelings of hopelessness. ?? To address suicide prevention holistically, we must move beyond individual interventions and focus on dismantling these root causes.?? This involves advocating for equitable policies that reduce economic disparities, ensure universal healthcare, and support housing, education, and employment opportunities for all.?????????? It also requires recognizing the importance of creating spaces that are affirming and inclusive of diverse identities, particularly for Mad and neurodivergent folk.?? It is always an honor to work with PESI because they allow me to examine the many intersections of my work and put them together for meaningful conversations within the mental health community. Monday, I was able to put on my death doula and grief counselor hat to talk about suicide and emphasize how mental health professionals can cultivate their practice to "create a space to hold the chaos" for our clients of LGBTQIA+ populations and those in other *marginalized communities.?The opportunity to think about the importance of inclusion is everywhere, especially as we think about lives we have lost, such as Dr. Antoinette “Bonnie” Candia-Bailey. We, as mental health professionals, need to understand how the weight of oppression and discrimination is causing some of us to find escapes, hope, and a validated place for us to be us, even through death. Instead, I am using "those experiencing lack of intentional political investment" to place the blame on the systems we exist in and the intentional lack of commitment to a group of people. Why Our Incarcerated Youth Deserve Equitable Mental Healthcare It’s time we talk about our kids behind bars. Did you know that up to 70% of these young souls are battling serious mental health issues? That’s right, almost three-quarters of them need real, therapeutic support, not just punishment. When we turn our backs on their mental health, we turn our backs on their future. Many of these young people have faced unimaginable trauma—abuse, violence, neglect. They’ve been through it all. Their actions often scream for help, but instead of getting the therapy they need, they get locked up. It’s like putting a band-aid on a broken bone. But here’s the real deal: when these kids get the mental health care they deserve, they start to thrive. Studies show that with proper treatment, they’re less likely to end up back in the system. It’s a cycle we can break. They start performing better in school, building healthier relationships, and dreaming big again. United Against Fentanyl Families and communities can play a vital role in fighting fentanyl’s devastating impact, which claimed over 70,000 American lives in 2022 alone, marking it as the leading cause of death for adults aged 18-45. Coming together means creating spaces for open dialogue, learning about addiction and harm reduction, and supporting those affected with compassion rather than judgment. We can strengthen community bonds by encouraging safe conversations about drug use, warning of fentanyl’s hidden dangers, and pushing for resources like Narcan, treatment options, and mental health support. Families can lead by example, educating loved ones, recognizing warning signs, and fostering resilience. By standing united, we amplify our voices for change and offer hope to those at risk, reminding everyone that no one has to face this fight alone. Together, we can advocate for policies and programs that make a real difference, helping to turn the tide on this crisis.

There is a thin line between criminogenic and mental health behaviors. It is shameful what takes place in many jurisdictions with youth. Many find themselves in circumstances and detention centers through no fault of their own. But merely representing the collective failures of their parents, schools and community resources. The question is always ‘what happened to you?’ Detention centers become the dumping grounds of our inability to address chronic mental health diagnoses. The school to prison pipeline is a real thing. When schools lack resources to address mental health for youth, the courts often finds themselves as a back stop. Residential treatment facilities have been dwindling over the years. Accordingly, detention centers, department of corrections and prisons become the default placement for youth with mental health diagnoses. We must do better.

We know that the lives of families and friends who have lost a loved one to suicide have been tragically altered, and our hearts go out to them.? That’s why in September, which is Suicide Prevention Month, it’s important to me to highlight the four signs that can help people spot someone who is in mental distress.? These are important – and helpful - to know, because you, or someone you know, could take action to help someone, especially a young person, facing serious mental health problems.?? ? According to the Centers for Disease Control and Prevention, suicide rates increased approximately 36% between 2000–2022 and suicide was responsible for 49,476 deaths in 2022 alone - about one death every 11 minutes.??? ? In the communities that Blue Shield Promise Health Plan serves, we work with many fine organizations that offer vital mental health services and support to young and old alike.? In San Diego, these include La Maestra, Family Health Centers of San Diego, San Ysidro Health, San Diego Family Care, and Rady Children’s Hospital. ? In Long Beach, organizations like Tri-City Mental Health, YWCA of San Gabriel Valley and Helpline Youth Counseling provide valuable mental health services to diverse communities.

Together with the Children and Young People's Mental Health Coalition (CYPMHC) and 87 other members we are calling on the Government for more urgent and wide scale action and investment in the children’s mental health system. The first budget from this Government was a crucial opportunity to demonstrate their commitment to raising the healthiest generation of children and achieving parity of esteem. There were some glimmers of hope, but more action is needed. We need to see: - Full roll out of Mental Health Support Teams - Young Futures Hubs in every community focused on providing early mental health support - Action to address the root causes of mental health - Increased investment in the children’s mental health system As a sector we are here and ready to support. The Government must use the Spring Budget to rest and truly invest in the mental health system for all babies, children and young people. It is been weighing on my heart whenever news of a life lost to suicide surfaces, prompting me to consider the significant toll it takes each year. It leads me to ponder: ???Were the key signs missed by family, friends, or society? ???Did those struggling with suicidal thoughts lack someone to confide in? ???Is society failing to foster an environment where seeking help is encouraged? ?These reflections lead me to ask certain questions about our roles as mental health advocates, such as: ???Are we falling short in our efforts? ???Are our preparations and policies not up to par? ???Are we not using the right tools in this battle? ???Are there inadequate resources in this field? ???Are we not effectively utilizing the appropriate platforms for advocacy? Let us contemplate these questions as we work towards enhancing mental health awareness and suicide prevention. ?? ?? I anticipate a future where recording any suicide cases within a year becomes a thing of the past. ?? For anyone who feels like giving up, it is very important to reach out for support because we are not alone in this journey. ??
