Our recent study, published in the Archives of Orthopaedic & Trauma Surgery, compared two surgical methods for treating ankle syndesmosis injuries: suture button (SB) and syndesmotic screw (SS) fixation. We found that the patients who received SB fixation reported better long-term outcomes based on patient-reported scores. The SB group also had a lower rate of complications such as implant failure, removal, and malreduction. Registry Study, Level III evidence, large cohort comparison using large database (national) Published online in JSES, to be published in print within the next weeks. Take-home messages: By 6 years after surgery in patients <55 (indication for surgery: primary glenohumeral osteoarthritis): 14.1% of hemiarthroplasty don't survive. 12.4% of stemmed anatomic TSA don't survive. 7% of stemless anatomic TSA don't survive. 6.5% of reverse shoulder arthroplasty don't survive. The trend is now moving towards implanting stemless humeral components in anatomic total replacements. They are twice as durable as traditional humeral components. The other trend, the "reversomania", seems to be durable as well! The habit of considering young age as a default indication for hemiarthroplasty has to come to an end! 19.8% don't survive at 8 years. High rate of painful glenoid erosion, especially in the younger active population. This makes a future reverse replacement very challenging due to glenoid bone loss. My rationale towards replacing an arthritic shoulder of someone <55 with normal cuff depends on the degree of degenerative bone changes: low degree: stemless anatomic. High degree: A BIO-RSA reverse! Bone grafting a glenoid in a patient<55 builds up bone stock for future revisions I don't trust hemiarthroplasty in a young patient. Most studies show that it is functionally much INFERIOR to total replacements. Plus it causes a lot of pain after the second post-op year mark.It rubs the glenoid surface vigorously. I don't offer it as an option. "First do no harm"
