Continuity of care can be difficult for healthcare organizations to provide when patients visit providers from multiple organizations. MEDITECH seeks to address this issue with the Traverse Exchange solution, a cloud-based interoperability solution meant to decrease the administrative burden required to reenter and manually share data. This report offers a detailed view of how MEDITECH’s data exchange network is improving care and alleviating data-sharing burdens for shared patient populations and providers in Canada.
