The American Academy of Allergy, Asthma and Immunology - AAAAI and the American College of Allergy, Asthma and Immunology (ACAAI) Joint Task Force recently released updated guidelines for AD management, encompassing several of the latest medications and addressing five crucial questions related to AD management: 1. Among patients with AD, what topical treatments should be used to achieve optimal outcomes? 2. Should elimination diets (dietary avoidance strategies) be used for AD? 3. Should dilute bleach baths be used for AD? 4. Should allergen immunotherapy be used for AD? 5. Among patients with AD, what systemic treatments, including phototherapy (UV light therapy), should be used to achieve optimal outcomes? ?? Publication alert: ESCD position paper on patch testing ?? Patch testing is the only clinically applicable diagnostic method for Type IV allergy. The availability of Type IV patch test (PT) allergens in Europe, however, is currently scarce. This severely compromises adequate diagnostics of contact allergy, leading to serious consequences for the affected patients. ? Against this background, the European Society of Contact Dermatitis (ESCD) has created a task force (TF) (i) to explore the current availability of PT substances in different member states, (ii) to highlight some of the unique characteristics of Type IV vs. other allergens and (iii) to suggest ways forward to promote and ensure availability of high-quality patch testing substances for the diagnosis of Type IV allergies throughout Europe. ? The suggestions of the TF on how to improve the availability of PT allergens are supported by the ESCD, the European Academy of Allergy and Clinical Immunology, and the European Academy of Dermatology and Venereology and intend to provide potential means to resolve the present medical crisis. Understanding Glucocorticoids in Acute Urticaria: A Clinical Perspective Acute urticaria, or sudden-onset hives, is a frequent challenge in urgent care and emergency settings. But when it comes to the role of glucocorticoids (steroids), recent evidence points to an intriguing finding: their benefit remains unclear. Two systematic reviews from 2024 have put the spotlight on this common practice: 1. Jamjanya et al. in the Journal of Allergy and Clinical Immunology concluded that parenteral antihistamines are the most effective intervention, while the added value of glucocorticoids wasn’t definitive. 2. Badloe et al. in the Journal of the European Academy of Dermatology and Venereology echoed similar findings, casting doubt on the routine use of glucocorticoids. What does this mean for our clinical practice? ?? Key Takeaway: Nonsedating second-generation antihistamines should be the first-line therapy for acute urticaria. Glucocorticoids should be reserved for specific situations—such as prominent angioedema or persistent symptoms despite several days of antihistamine therapy. ?? New publication alert: High content of hexavalent chromium in cement samples from three African countries (https://lnkd.in/e-Gv3bTb) ?? Hexavalent chromium is an important contact allergen traditionally found in cement This study showed that in six of eight cement samples from three African countries, the Cr(VI) content was ≥16 mg/kg, more than eight times the maximal allowed concentration in the EU. This finding suggests that high hexavalent chromium concentrations in cement in three African countries may contribute to the high prevalence of chromium contact allergy seen in these regions. Regulating the allowed levels of contact allergens in commonly used substances is key to preventing contact allergy across the world!

Our review article was published in Current Opinion in Allergy and Clinical Immunology. The highlights of the review are as follow: ~Stevens–Johnson syndrome/toxic epidermal necrolysis (SJS/TEN) are severe cutaneous adverse drug reactions characterized by widespread epithelial detachment and blistering, which affects the skin and mucocutaneous membranes. ~ No targeted therapies for SJS/TEN currently exist and so the mainstay of management remains supportive care. ~Though our understanding of the pathogenesis of SJS/TEN has advanced from both an immunological and dermatological perspective, this knowledge is yet to translate into the development of new targeted therapies. The key points are: ~ A greater understanding of SJS/TEN pathogenesis will unlock novel therapeutic options and predisposing risk factors. ~ Advanced skin culture models will facilitate understanding and provide screening tools for assessing SJS/TEN therapeutics. ~ Significant inter-individual variability exists in SJS/TEN. I am ecstatic to share this new story from my lab, which was helmed by Cameron Flayer, Ph.D.! https://lnkd.in/gTteJhWQ A few years ago my lab discovered that the sensory nervous system - and not the immune system - directly detects protease allergens in na?ve skin leading to an itch response and the release of a neuropeptide Substance P. This activates allergic-skewing dendritic cells to migrate to the draining lymph node and start up the entire immune response. But if we all have sensory neurons and if we are all surrounded by the same amount of allergens, why aren’t we all allergic? We hypothesized that an innate immune cell might actively prime neurons to respond to allergens by setting their threshold of reactivity. This idea was inspired by work from Brian S. Kim's group showing that immune cytokines could prime the itch response in sensory neurons to histamine. Using a behavioral screen, Cameron Flayer, Ph.D. found that gamma/delta T cells were required for the immediate itch response to wide ranging protease allergens (including mosquito saliva). This priming function was specific to a poorly described epidermal gamma/delta cell population that we abbreviated as GD3. GD3s made IL-3 under homeostatic conditions, which acted upon PEP1 neurons via their expression of the IL-3 receptor. Downstream of the IL-3R, IL-3 stimulation led to JAK2-mediated priming of allergen-induced itch, and STAT5-mediated production of Substance P and downstream dendritic cell migration and initiation of allergic immunity. Ultimately, we think that GD3 cells and their IL-3 act as part of a neuroimmune rheostat involved in allergic immunity. Ramp it up – develop hyper-responsiveness to allergens. Tone it down – develop a state of ignorance to allergens. Happy to share our study showing that extracellular vesicles and their miRNA content contribute to NSAID-exacerbated respiratory disease (N-ERD). N-ERD is a severe type 2 chronic inflammatory disease, characterized by nasal polyposis, asthma and intolerance to NSAIDs such as Aspirin. In this study, published in Allergy, Franziska Hartung, Pascal Haimerl and colleagues show that small vesicles found in the airways of N-ERD patients modulate macrophage activation, resulting in reduced immune functions that are important for clearance of bacterial or viral pathogens. Our study thus identifies a new role for extracellular vesicles in type 2 airway inflammation and may explain why N-ERD patients are susceptible to infection-induced exacerbations.
