Today, we join the effort to increase awareness and support for Pediatric Acute-onset Neuropsychiatric Syndromes (PANS) and Pediatric Autoimmune Neuropsychiatric Disorders Associated with Streptococcal Infections (PANDAS). Repost from beyondpans ? PANDAS/PANS is (in huge part) a pediatric disease. There is good news and bad news here. Bad news: Onset at a younger age makes it harder to diagnose. Doctors often tell you that all these symptoms (emotional outbursts, hyperactivity, mood swings, food refusal…) are just behavioral and typical for the age. Or they say you need to discipline the child better (that’s more or less what we were told at the pediatric neurology department, despite severely skewed EEG results and many symptoms that could not be described as behavioral. Also, often the worst symptoms for the child are those ‘inside their head’ (their brain is ‘on fire’), and they can’t express themselves verbally at a young age. During flares, they may go through intense mental states, intrusive thoughts and images, hallucinations, and even psychosis. But the way it shows up is screaming, hitting their head, or aggression. That’s why even parents sometimes miss the sudden onset part. Or they see the sudden change in the child as a reaction to some life event (moving houses, new school, divorce…) Good news: If diagnosed and treated early, the chances of recovery are much higher. Around this age, both the immune system and the nervous system are not fully developed yet, and you can do a lot to rebalance these, but you need medical treatment. You need a knowledgeable doctor to diagnose it, order diagnostic tests, and prescribe the right medication (such as antibiotics, antivirals, antihistamines, antiinflammatory drugs etc.) That’s why we need more awareness among the doctors and the general public. I'm proud to announce that our article 'GPs’ perspectives on diagnostic testing in children with persistent non-specific symptoms.' has been published in British Journal of General Practice. This study explored General Practioners' (GPs) motives when making decisions about diagnostic testing in children. This research showed that the decision to conduct diagnostic testing in children with persistent non-specific symptoms was based on a complex trade-off among medical considerations, psychosocial factors, consultation management and efficient resource utilization. The study demonstrated that although GPs wanted to limit unnecessary invasive procedures in children, non-diagnostic motives for testing were considered important, such as providing reassurance or securing the GP-patient relationship. This publication highlights the importance of awareness and personal motivation in decision-making for diagnostic testing. This is the first study to explore these motives in children with persistent non-specific symptoms. Greater awareness of these motives could lead to changes in testing practices and help reduce both undertesting and overtesting in children. Seizures are one of the hallmarks of KCNQ2, with nearly all of those affected experiencing them in the first week of life. The most common seizures experienced are focal tonic, resulting in stiffening of the body, with autonomic features, such as changes in breathing and heart rate, and colour in the face. KCNQ2 pathogenic variants that result in symptoms on the mild end of the spectrum, specifically benign familial neonatal epilepsy (BFNE), are typically inherited from a parent with the same variant. In such cases, there is often a family history of neonatal seizures that spontaneously resolve.
