A very important study and an accompanying editorial discuss how escalation of hypertension treatment may add to the risks of falls and major fractures for older people in nursing homes. Though, there are many proven benefits of tight BP control, if they really all apply to older and frail nursing home patients, is not clear. So, much caution should be used when aiming to aggressively control BP among frail NH patients. Here is the advise from the editorial: "In a perfect world, orthostatic vitals would be monitored around changes in blood pressure management... Vital signs could be monitored more frequently, at least with each shift change.... health care practitioners at nursing homes might avoid rapid intensification of antihypertensive medication—perhaps allowing at least a month for acclimation to a new dose or drug... And, when initiating a new medication, begin with the lowest dose available." To help manage these risks, Dr. Shukla emphasized the importance of asking patients to bring all their medications—including prescription drugs, over-the-counter medicines, and supplements—when visiting healthcare providers. Nurses should then compile a comprehensive list of the medications the patient is taking. This list allows them to: Identify repetition of medications, which may occur when different brand names contain the same active ingredients, leading to unintentional duplication. Check the chemical composition of each medication to ensure no dangerous interactions are occurring. Ensure all medications are necessary and appropriate for the patient’s condition. By following these steps, nurses can help prevent medication errors, improve patient safety, and optimize treatment outcomes.
