Care Transitions in Kidney Transplant. The 3-Item Care Transition Measure (CTM-3) is a quality metric in the proposed Increasing Organ Transplant Access (IOTA) Model by the Centers for Medicare & Medicaid Services (CMS). This measure is designed to evaluate the quality of care transitions from hospital to home for patients who received a kidney transplant, focusing on their preparedness to manage their health post-discharge. The relevance of CTM-3 within the IOTA model directly correlates with the model's objectives to enhance care coordination, improve patient outcomes, and increase access to kidney transplants. By assessing how well patients understand and are prepared for their self-care and medication management after leaving the hospital, transplant centers collecting and analyzing patient responses to CTM-3 aims to reduce unplanned hospital readmissions, a critical factor in both improving patient quality of life and reducing Medicare expenditures. The key - to perform well in the IOTA model transplant centers would not just need to collect this data but be capable of evaluating it and deploying interventions to support patient care in real time.

We need to address an industry that should be a beacon of hope for those battling chronic kidney disease but has instead become a maze of disjointed efforts, fractured care, and neglected responsibilities. The dialysis industry in the United States should focus on patients’ holistic well-being but has allowed itself to be consumed by lab values and numbers—ignoring the broader, more humane aspects of care From outdated chairs in clinics to the uncleanliness that some patients endure, these “small” things are symptomatic of a larger issue. The lack of consistent education and awareness among patients makes it worse. How can patients advocate for themselves or understand their treatment options when the industry fails to provide them with the necessary knowledge? It’s time we shift our focus from merely managing lab results to valuing the entire patient—their comfort, understanding, and well-being. Holistic care is not just about treating a disease; it’s about treating a person with unique needs, fears, and hopes that deserve full attention and respect. Why do we celebrate the few who advocate for change yet remain silent when it comes to collective action? We cannot simply ‘like’ and ‘comment’ on social media posts about the industry’s failures and think our job is done. We must speak up, share experiences, and unite our voices. Despite evident issues, why haven’t we seen progress? The answer lies in our fragmented approach to advocacy. Too often, we rely on a handful of vocal advocates while the majority remain silent, content with small improvements or afraid to challenge the status quo. But silence is complicity. If we want real change, we need a grassroots movement that unites patients, caregivers, healthcare professionals, and concerned citizens. We need to break down the barriers that large dialysis organizations. These organizations, with their influence, have long dictated the narrative, convincing many that the current system is as good as it gets. But we know better. We know innovation and patient-centered care are possible when smaller, independent voices flourish. The time to act is now. We cannot continue to accept the status quo. We must demand better technology, education, and care for those whose lives depend on dialysis. Let us remember that our collective voice has the power to enact change. We must be relentless in our pursuit of a better future for dialysis patients—a future where care is holistic, where every detail, no matter how small, is attended to with the respect and dignity every human being deserves. I urge you to reflect on the role you can play in this fight. Whether you are a patient, a healthcare provider, or an advocate, your voice matters. Only through united efforts can we dismantle the egregious policies that have plagued this industry for too long. Let us not wait for someone else to lead. Let us be the change-makers who refuse to accept anything less than the best care for every dialysis patient.

It’s high time we confronted the reality of how the two major dialysis corporations are feeding off the vulnerabilities of #CKD patients. Instead of investing in genuine, life-extending options like prevention, early intervention, or support for kidney transplants, these organizations are profiting by keeping patients tied to a grueling #dialysis regimen. Research shows that long-term dialysis isn’t just exhausting; it can significantly reduce life expectancy. A 2021 study published in the American Journal of Kidney Diseases found that the average life expectancy for a dialysis patient is often five to ten years, with survival rates sharply decreasing over time, especially for those who start dialysis at an older age or with other health complications. Yet, kidney transplants, when successful, can dramatically increase survival rates, giving patients a new lease on life with far fewer health complications. Transplant recipients typically live up to 10–15 years longer than those who remain on dialysis. Despite this, the big players in the dialysis industry rarely, if ever, promote transplants as a viable first-line treatment or a better long-term option. Why? Because every transplant means one less customer, one less profit stream. The fact is, transplants don’t benefit these companies’ bottom lines. By keeping patients dependent on dialysis, they create a steady income from each treatment session, multiple times a week, every single week. Dialysis is the cash cow, and these corporations are milking it for all it’s worth Beyond this, prevention is practically nonexistent in their approach. Educating the public about how to reduce the risk of kidney disease, or even funding research on ways to reverse kidney damage, is not profitable in a model that relies on chronic treatment. Their public messaging avoids any real push for preventative health measures or awareness campaigns that could make a difference. This lack of transparency and patient-centered focus goes against every ethical standard in healthcare. Instead of using their resources to genuinely improve patient outcomes or promote kidney health, they focus on securing repeat customers. Real, patient-centered care would involve promoting awareness around kidney health, providing better access to information on transplant options, and actively supporting preventive strategies to avoid dialysis in the first place. But we don’t see that. What we see are campaigns that normalize dialysis as the “only option” and keep patients and their families unaware of alternatives. The data is clear, but these corporations choose to ignore it because it doesn’t serve their financial interests. Every year, thousands of patients are forced onto dialysis without understanding the toll it will take on their lives or knowing that there are better, less exploitative options.
