A groundbreaking study has shed light on the cardiac, vascular, and metabolic changes in young adults who survived childhood cancer. Conducted by a team of researchers including Olof Broberg, Ingrid ?ra, Thomas Wiebe, Constance Weismann, Petru Liuba, the study found significant differences in cardiovascular health markers between survivors and healthy controls. ?? Utilizing advanced ultrasonography, researchers discovered that childhood cancer survivors, particularly those exposed to higher doses of anthracyclines, exhibit critical changes in carotid distensibility and endothelial function. These survivors also showed higher levels of atherogenic apolipoproteins and lipids, factors contributing to an increased risk of cardiovascular diseases (CVD) later in life. ?? Thyroid Cancer A Review JAMA. 2024;331(5):425-435. doi:10.1001/jama.2023.26348 Abstract Importance Approximately 43?720 new cases of thyroid carcinoma are expected to be diagnosed in 2023 in the US. Five-year relative survival is approximately 98.5%. This review summarizes current evidence regarding pathophysiology, diagnosis, and management of early-stage and advanced thyroid cancer. Observations Papillary thyroid cancer accounts for approximately 84% of all thyroid cancers. Papillary, follicular (≈4%), and oncocytic (≈2%) forms arise from thyroid follicular cells and are termed well-differentiated thyroid cancer. Aggressive forms of follicular cell-derived thyroid cancer are poorly differentiated thyroid cancer (≈5%) and anaplastic thyroid cancer (≈1%). Medullary thyroid cancer (≈4%) arises from parafollicular C cells. Most cases of well-differentiated thyroid cancer are asymptomatic and detected during physical examination or incidentally found on diagnostic imaging studies. For microcarcinomas (≤1 cm), observation without surgical resection can be considered. For tumors larger than 1 cm with or without lymph node metastases, surgery with or without radioactive iodine is curative in most cases. Surgical resection is the preferred approach for patients with recurrent locoregional disease. For metastatic disease, surgical resection or stereotactic body irradiation is favored over systemic therapy (eg, lenvatinib, dabrafenib). Antiangiogenic multikinase inhibitors (eg, sorafenib, lenvatinib, cabozantinib) are approved for thyroid cancer that does not respond to radioactive iodine, with response rates 12% to 65%. Targeted therapies such as dabrafenib and selpercatinib are directed to genetic mutations (BRAF, RET, NTRK, MEK) that give rise to thyroid cancer and are used in patients with advanced thyroid carcinoma. Conclusions Approximately 44?000 new cases of thyroid cancer are diagnosed each year in the US, with a 5-year relative survival of 98.5%. Surgery is curative in most cases of well-differentiated thyroid cancer. Radioactive iodine treatment after surgery improves overall survival in patients at high risk of recurrence. Antiangiogenic multikinase inhibitors and targeted therapies to genetic mutations that give rise to thyroid cancer are increasingly used in the treatment of metastatic disease. https://lnkd.in/gwzFk_Wj

Kudos to Elisa Martini, the first author, and the outstanding PIs, Gianluigi Condorelli and Marinos Kallikourdis, from Humanitas Research Hospital, on their groundbreaking publication in Circulation Research. ??This research unveils how pressure overload-induced heart failure partially fulfills criteria for autoimmune disease. The study identified novel cardiac self-antigens capable of triggering cardiac dysfunction. Even more exciting, they developed a tolerizing vaccine using these antigens, which reduced disease severity in mouse models through the induction of effector regulatory T cells. ?? Heart failure remains a significant global health burden, affecting over 64 million people worldwide. Promising research in Nature CD45-PET: a non-invasive tool to image inflammation Inflammation happens in a range of diseases - autoimmunity, aging, cancer. In this study authors used CD45 (which is expressed only by immune cells and by broadly most immune cell subtypes) as a marker of inflammation combined with positron emission tomography (PET). CD45-PET imaging demonstrated exceptional sensitivity and clarity in detecting inflammation across diverse preclinical models, surpassing 18F-FDG-PET in accurately reflecting disease severity in lung and bowel diseases. A human CD45-PET was also developed for clinical translation. The Journal of the National Cancer Institute just published an article regarding the racial disparities in colorectal cancer incidence and mortality between Black and White Americans and how they relate to surveillance colonoscopies. The results speak loudly to me both as a healthcare provider and as a medical science liaison. ??? The article, titled “Impact of racial disparities in follow-up and quality of colonoscopy on colorectal cancer outcomes” (linked below), found that eliminating disparities between Black and White patients for follow-up colonoscopy rates and quality of colonoscopy screening would result in a 14.6% reduction in colorectal cancer incidence, an 18.7% reduction in colorectal cancer mortality and a 7.1% improvement in life-years gained for Black patients. ?? As a GI nurse practitioner ????, studies like this highlight the importance of taking intentional steps to ensure adherence to recommended surveillance colonoscopies for Black patients above and beyond the standard methods used for patient recalls. As a medical science liaison, it is a call to action to focus medical device innovation on eliminating disparities such as these and to have open conversations with physicians about how we can work together to ensure health equality for all patients. ?? Being aware of these statistics isn’t enough, it is our responsibility in all sectors of healthcare to take action and bring measurable change to improve patient outcomes. The researchers, led by Brian D. Nicholson used electronic health records data to look for people diagnosed with cancer within six months after a recording of unexpected weight loss. ? The original study included nearly 64,000 patients with unexpected weight loss recorded in their medical records between 2000-2012, of which 1.4% ended up receiving a cancer diagnosis within six months. In the update, the authors analyzed records from about 326,000 patients with unexpected weight loss between 2000-2019. This time, 4.8% had received a cancer diagnosis half a year later.
