Harvard’s Dr. Robert Green said he thinks more lives would be saved through genetic sequencing for single gene mutations compared with the type of polygenic screening that MassMutual is undertaking. Polygenic screening generally does not look for mutations in the BRCA1 and BRCA2 genes, for example, which increase the likelihood of developing breast cancer. “Direct-to-consumer testing has been great for genetics in that it has democratized and demystified genetics,’’ Dr. Green said, “but in general, they do not look for the thousands of other gene mutations you could find if you sequenced.’’ The conditions covered under the initiative are atrial fibrillation, breast or prostate cancer, cardiovascular disease, high blood pressure, high low-density lipoprotein cholesterol, low bone density, and type 2 diabetes. 4DISCUSSION: German Precision Medicine Model Project Gets Ready for Kickoff as Commercial Providers Cry Foul NEW YORK – Following the start of the European soccer championship in Germany last week, the country is preparing to kick off another large endeavor next month: its national pilot project for genomics-guided precision medicine. Officially named the "Model Project for Comprehensive Diagnostics and Therapy Selection by Means of Genome Sequencing for Rare and Oncological Diseases," the project is supported with €700 million ($749 million) from the German public health insurance system over five years. Long in preparation, it plans to test the feasibility and utility of decentralized genomic testing in medical care across the country, while collecting data for research and to improve patient care. Participation in the project has been restricted to university hospitals and their sequencing facilities, leading commercial diagnostic providers, which play an important role in clinical genetic testing in Germany, to fear they will lose out. In the meantime, hospitals have been building up their sequencing and data infrastructure for the project's start in July. In a paper published in JCO Oncology Practice the following question was posted to clinicians: "How much does knowledge of the patient's insurance influence your decision to obtain the testing?" Almost half of the doctors, 47%, said this is a very significant factor, meaning if the insurance doesn't cover it, perhaps they won't order the test even though it might be lifesaving. While this paper was published in 2017, as Maurie Markman, MD from City of Hope points out in this Medscape commentary, it remains a critical question today and is becoming even more important everyday as new targeted and tumor agnostic therapies become available.
