  • Population health is transforming quality measures into culture. Focusing on the patient experience is challenging the healthcare industry’s traditional model of care. At the core of this evolution is data analysis and looking for answers outside the clinic setting, said Hannah Smith, Heritage Health's Population Health Manager. “Our mission as an organization is to improve life,” said Smith. “Our team is all about the patient outcomes. Our team is transforming quality measures into culture. We’re responsible for finding ways to support our patients through our care teams.” Population health acts as a support mechanism for clinical care teams who have close relationships with patients. Heritage Health has a five-member team that connects patients with resources. Roxanne Kilmartin, quality coordinator for Heritage Health, said the population health initiative has strategic priorities and analyzes the social drivers of healthcare. “We look at access to transportation, education and their financial stability,” said Kilmartin. “Essentially, we study where we work, where we live and where we play as a community.” “Patients are a person, and we must consider them in those terms. How do we make our care more person-centered? If you have a Type 2 Diabetic, who isn’t making any progress, maybe they don’t have the financial security to afford healthy food. We must look at the factors in their lives that might be holding them back.” Heritage Health serves roughly 27,000 patients annually in Kootenai, Shoshone and Benewah counties. These patients represent a broad socio-economic demographic of North Idaho residents. Tearing down assumptions about patients is a key component of the research. “We have to find ways of documenting the things that are happening in their lives,” said Kilmartin. “An example would be a patient who had chronic back issues that were we’re not being resolved. Nobody had thought to ask the patient what type of mattress they were sleeping on. It turns out the patient didn’t even have a bed to sleep in.” Collaboration between administrative teams like the Population Health Department Quality Department and clinical care teams is essential for improving patient experiences. “I started as a medical assistant and grew into the role I am in now,” said Smith. “My heart will always be in the clinic. Everything we do is with the patient in mind. Looking at data helps us make sound organizational decisions.” The population health initiative imitative also hopes to reduce provider burnout, which is a national issue, by providing support and solutions based on trust and data. “Patient-focused care does reduce burnout,” said Kilmartin. “It also results in better outcomes. The key part is acting on data. It’s not just documenting issues. We have to work proactively to find the answers for the patient.” -Written by Marc Stewart, Content Strategist for Heritage Health.

  • Heritage Health’s CEO Mike Baker and Chief Operation Officer Glenn Adams presented a $10,500 donation to the Kootenai County Fire Chief’s Association to help increase the survival rates of cardiac arrest victims. The donation will help fund PulsePoint, an interactive app that pairs CPR trained individuals, locations of emergency AEDs and people in cardiac arrest together. This very successful program is utilized all over the country and has increased the survival rate of cardiac arrest victims. “We’re honored to make this donation,” said Baker. “We exist to improve life and connecting people with CPR skills with people in need of lifesaving measures is a perfect match.” When bystander CPR is given to a cardiac arrest victim prior to first responder arrival, studies have shown an increase in survival of over 40%. Unfortunately, less than half of cardiac arrest victims receive bystander CPR. This program will save lives in Kootenai County.v
