OCTober satisfaction?????? ??♂? A 52-year-old asymptomatic man with several cardiovascular risk factors and incidental findings at a sport medical examination of electrocardiographic and echocardiographic alterations compatible with a previous anterior myocardial infarction, along with SPECT evidence of severe anterior perfusion deficit, presents to the cath lab with this haziness image in the proximal left anterior descending artery (LAD). ?? We decided to perform an OCT pullback, which proved to be highly educational from an imaging perspective! A clearly appreciable organized and recanalized thrombus with a small minimal luminal area (MLA) of 1 mm2, interesting also viewed in the longitudinal axis. Additionally, a small calcific nodule with a thin cap and a posterior shadow is visible in the most proximal tract of the vessel. ? We treated this lesion with a direct stenting, with a satisfactory final result, both angio and OCT (stent expansion 99%, no edges dissections, good stent strut apposition); started DAPT with ASA + Ticagrelor. ?? Key message: improved lesion characterization, better interventional and clinical treatment decision-making. This individualized strategy for multi-organ repair is poised to be a game-changer in resuscitation. CARL not only restores circulatory function but also incorporates a cytoprotective solution designed for comprehensive organ repair. With ongoing research funding and clinical trials, this innovation holds great potential for enhancing survival rates and long-term recovery after cardiac arrest.
