Do you want to see an org making a big impact in a community? (Surprise -- they're not even a healthcare provider or payer.) Check out Thrive Allen County's annual report! Kudos to Rural Health Information Hub for the excellent spotlight. This Kansas powerhouse non-profit is proof that a smaller population area doesn't have to mean a smaller impact. Thrive Allen County's secret sauce? Included but not limited to: 1) They share skills and resources in grant writing, strategic planning, and more. 2) They host community conversations and reinforce systems thinking for whole health in a community (physical, fiscal, social, emotional). 3) They dedicate resources to address stigma related to substance misuse and increase recovery housing. 4) They promote upcoming grant opportunities from wide pool of sources aligned with community needs. It's not just the historical sources either. 5) They work to build trust among stakeholders and unite talents to fuel innovation. It is clear that our current #health and #care system needs to be reformed. Increased focus on the social determinants of health is required to understand and address the underlying causes of poor health and #wellbeing in people’s lives, working from a model of #prevention rather than treatment. We had the pleasure of hosting Stephen Kinnock, Minister of State for Care, at the Centre today to showcase our innovative approach to support across health and wellbeing. The Bromley by Bow Centre is a hub for the community. Together with our partners, Bromley by Bow Health, we offer our #community a place and a space to come, be and connect. Our person-centred, #holistic, and #integrated support works because it builds on the relationships our community has with our Centre. Our message is simple: Only together can the Third Sector and the #NHS address the growing #HealthInequalities in our communities. Our health and care system must be based on community #empowerment. Community Centres must be included as decision makers for our country’s health and wellbeing. Our model needs long term, sustainable funding, in contrast to the current funding model available to us, so we can continue the work our community deserves. #CommunityHealth #Charity

In a survey conducted by ReThink Health, 82% state that, “Not enough funding for our services, programs or policies,” is a barrier contributing to equitable community well-being.* To meet the financial challenge faced by health care organizations, health care foundations are leaning in and incorporating health equity in their philanthropic case for support.
