In this episode of the IN THE MIND OF PODCAST, physical therapist Rob Shapiro, DPT, COMT, NKT and Donis Gil, ATC discuss the history and utility of using The Hierarchy of Rehabilitation and Athletic Development system to guide rehabilitation. They explain how the hierarchy gives clinicians a framework to systematically assess patients, identify deficits, and progress treatment. Rob and Donis trace the origins of the hierarchy approach starting with strength coach Al Vermeil's athletic pyramid which was then adapted to rehab by Rob Panariello, MS, PT, ATC. They discuss how it was further modified for rehabilitation by incorporating pain management and evaluation components. The hosts talk through the value of having a basic algorithm to lean on, especially for new clinicians. They emphasize that the hierarchy allows clinicians to utilize their preferred treatment techniques while following a consistent thought process. Rob and Donis reflect on the hierarchy's role in improving critical thinking and preventing missed steps in treatment. #rehabhierarchy #physicaltherapy #pt #athletictraining #sportsrehab #painmanagement #treatmentprogression #rehablevels #rehabpyramid #criticalthinking #rehabframework #rehabalgorithm #rehabsteps #systematicrehab #rehabthoughtprocess https://lnkd.in/ewKgh_r3

Recent Revive Stronger podcast with Samuel Buckner, Holly Baxter and Josh Pelland on how much volume is optimal for muscle growth. We go deeep into this one based on a recent critical paper by Sam & Holly that raised some interesting points, including skepticism about the outlying results of Brad Schoenfeld's lab (which I'm personally not concerned about). Lyle of course quickly chimed in. After talking to Brad, he rightfully noted that data collection of our rest interval paper was actually not even conducted at Brad's lab but at Oklahoma University, so Brad didn't do the ultrasound scans. Also deep dives into: - How much of the effects of high volumes can be attributed to mere muscle swelling? (Josh had particularly great insights on this one.) - What are our practical recommendations? I appreciate Whitney Lowe ‘s forward-thinking interpretation if the research study on manual therapy. (Because research papers are never my thing!) I’m so excited to be part of a forward thinking community, that also is rooted in centuries old fundamental understanding of the power our physical touch can have. My favorite point of Whitney Lowe commentary was how not only our touch -but our words and how we talk about the client’s body -can also have a very long term effect towards their opinion of their current condition. What we say about posture or even imaging, can have a nocebo affect. My goal with clients is to help them find the balance within themself. Never to compare their body against another body. Together we explore, with curiously, what is bothering them. Their concern is to be validated -without judgement-and if my instinct/knowledge/ technique is not a clear line to their concern I share my line of reasoning so we are both on the same journey towards balance.

In the context of Muscle Activation Techniques, palpating refers to the act of using touch or pressure with the hands to feel and assess the condition, tension, or responsiveness of muscles. Practitioners like Bill use palpation as a hands-on method to identify areas of muscle tightness, weakness, or imbalance. By palpating, they can gather information about the muscle's texture, temperature, and sensitivity, which helps guide them in applying specific techniques to improve muscle function, relieve tension, or address imbalances in the body. Palpation is a valuable skill and it aids in the assessment and treatment of musculoskeletal issues with our members. Want to hear more? The more we take sides the more entangled we get. The more we try and make sense of things, the more it drains our energy. The solution, people, in one word is: Lung! Breathing techniques activate amazing lung functions (Chinese medicine) that naturally rejuvenate and keep us in the here and now, and there’s nothing better than feeling free of the past, not worried about the future, and liberated from that urge to judge or scream at someone, or shove an opinion down someone’s throat. When everyone is reactive it’s impossible to talk to make sense of things, so we really need to use internal resources to make sense of ourself first. The takeaway this week; if we don’t feel comfortable in our skin, we can’t be ourselves and then we aren’t at peace. Next thing we’re entangled and triggered, and picking sides to try and make sense of the insensible. Silence, stillness and self-awareness are the cure, to create some space to regroup, reboot, and re-establish strong foundations. We can’t talk or think our way out of this, we must choose to?act differently. So, take a breath…and start. This is the theme of episode 40 of the FutureChi podcast, that I do with my pod partner Leon Fitzpatrick. Check out what happens when a Daoist and a designer get talking.
