This is the exact same model as "community policing". Cut the original, pretty effective public service to the bone under the guise of "saving money for taxpayers", remove the qualified and highly trained individuals, and then upon public uproar when critical skills, knowledge, and social context is lost and the general public suffers, "make amends" by putting in associates with less training to make up the deficit... where they carry almost as much responsibility, with far less of the authority, power, and knowledge required, and often at a higher cost than the originals (who were "cut for cost" in the first place). This gross mismanagement - and, potentially, negligence - isn't fair or safe for the public, or for the associates, and obfuscates the fundamental question: Since our taxes are high and we still pay them... where have they gone, if not here?

Throughout my career I’ve had the opportunity to get hands on and familiar with various types of video laryngoscopes (VL) utilised in airway management. From the compact and versatile McGrath Mac, which became the go-to device for intubations at one hospital I worked at, to the various versions of GlideScope and CMAC devices that have evolved over time to become more portable and useful in the pre hospital environment. I’ve observed the significant advantages of VL in improved visualisation of airway anatomy, especially important during Rapid Sequence Intubation (RSI) and crash intubations. It’s great to see many ambulance services are now incorporating VL into their practice for these critical procedures. By integrating the principles DASH-1A (Definitive Airway Without Hypoxia on the First Attempt) and a standardized approach to airway management, which includes the use of VL, a standardized airway kit, checklists, and appropriate medications to manage the physiological effects during laryngoscopy, I believe we can significantly improve success rates. However I also believe that adopting a quality improvement process is also crucial to helping with the overall goals of DASH 1A. One feature I find absolutely useful, is the ability to record video of each intubation attempt with the CMAC S VL we use at Ornge. This capability allows us to upload the footage to a secure server for review by our physicians, facilitating continuous skill improvement through 1 on 1 debrief after airway attempt with our Doctors and clinical educators. I am keen to learn about the practices and protocols other services utilize for their RSI/ETI clinical practice guidelines (CPGs) and quality improvement programs. How are you leveraging VL in your airway management strategies?

In September I celebrated 1 year at Novartis and I couldn't have summarized my experience better than a post by Viet Le. A few things I've learned along my journey: 1. Feeling seen: the ability to show up as an authentic leader and contributor at a workplace that values diversity, innovation, being unbossed will propel individuals to perform at peak and for a greater cause: patients. 2. Navigating adversity: how a workplace rallies around not just professional but personal adversities demonstrates their investment in you. Sometimes, it is not about solving complex problems at work but about how I can get creative to coordinate work travel while being a mom and making sure my 2 year old still gets fed, picked up from daycare, etc. It takes a village and you realize how important work can be a significant part of that. 3. Collaboration: when we can all come together and align on simple questions such as “what is our shared priority” and “how do we better the lives of patients”, magic happens! We get creative, we embrace challenges, we leverage data, and we look at ways to change human behavior to improve care quality and outcomes. I'm looking forward to learning and leaning in on my journey here at Novartis.
