Thank you for sharing your insights in our poll regarding how you educate your physicians in improving their documentation! For over 20 years, health systems and hospitals have dedicated efforts to educating physicians in clinical documentation to accurately reflect patient severity reporting. Despite these efforts, there seems to be minimal long-term impact on physician documentation practices. This is evidenced by the most common queries and the query rates on those common conditions not declining over time. Most organizations employ a mix of strategies from the poll to educate physicians, encountering recurring challenges like scheduling difficulties, low participation rates, and reduced interaction between CDI teams and physicians, especially as CDI teams increasingly work remotely. Moreover, assessing the effectiveness of such education remains a challenge; initial improvements may lead to a temporary decrease in queries, but sustaining these improvements has been elusive. Without credible data, it is simply not possible to assess the impact of your education efforts as it pertains to affecting long-term change in documentation practices.? ClinIntell has found that sharing population-based statistics with physicians on an ongoing basis alongside readily accessible training and documentation tips leads to sustainable impact.
