Congratulations to the Yale PERT! This is well-deserved recognition that has been years in the making! https://lnkd.in/dTMZpPCX I’ve had the unique experience to support 500+ PE thrombectomy cases with this world-class team! It never gets old seeing patients who need treatment receive the appropriate care in a timely fashion and improve immediately!???? Many people look at my photo stream and get grossed out by the many “clot maps” like the attached pic. The clot removed from the pulmonary arteries is just a visual to quantify the blockages straining the heart. Forget the clot, the numbers tell the story…the numbers matter! This patient, like many might be missed because they had “normal blood pressure” and did not require O2 support (yet). Despite looking “OK on the surface”, they were in cardiogenic shock as measured by their low Cardiac Index of 1.7. Because they were diagnosed, triaged, and treated by Yale’s Pulmonary Embolism Response Team (PERT) quickly, the patient’s vitals turned around on the table…immediately: ??decreased pulmonary artery pressures ??drecreased heart rate ??increased blood pressure ??cardiac output ??cardiac index …the pre treatment numbers indicated that this patient was likely heading in the wrong direction, however, following the Inari Medical FlowTriever Thrombectomy System’s final suction of the pulmonary artery clogging clot, the numbers were already ??!

Fluid resuscitation is a crucial aspect of emergency and critical care. Since the advent of the concept of early goal-directed therapy, we have placed a huge emphasis on aggressive fluid resuscitation in patients with severe sepsis and septic shock. From EGDT to PROCESS/ARISE/PROMISE to Surviving Sepsis Guidelines, we have seen a shift in how fluid resuscitation is monitored, but the idea of aggressive fluid resuscitation is still the crux of our hemodynamic management of these patients. Yet, the FENICE study showed that in 46 countries, there is a “huge variability in the current practice regarding an FC [fluid challenge]…and may reflect the controversies in current guidelines.” (Cecconi 2015)
