On this Rare Disease Day, I wish to share a perspective close to my heart, shaped by my experiences working closely with patients while being a part of the industry. As highlighted by ISPOR—The Professional Society for Health Economics and Outcomes Research (HEOR), patient-centricity stands among the top 10 trends for 2024-25. Placing patients at the core of healthcare interventions is not just a trend; it's an imperative. By prioritizing the patient experience, we unlock numerous benefits, from understanding the intricacies of the patient journey to identifying preferences and defining treatment success. This systematic review examines the efficacy and safety of risk stratification tools for predicting healthcare utilization in primary care settings, revealing a gap between model performance and real-world outcomes. 1?? Half of the identified models underwent external validation, with only two models validated internationally. 2?? Most real-world studies showed no significant benefit or even increased healthcare utilization from interventions guided by these models. 3?? Predictive performance was robust across different contexts, but real-world application did not consistently translate to improved outcomes. 4??
