DIGI4Care’s busy Year Kicks Off with the 3rd Interregional Partner Meeting ?? Cardiovascular disease, diabetes, and cognitive impairments are among Europe’s most pressing healthcare challenges, particularly for its aging population. DIGI4Care is committed to: ?? Redefining patient pathways by testing, validating, and integrating digital solutions through four pilot actions. ??Fostering collaboration, knowledge sharing, and innovation capacity. ??Bridging the digital divide between newer and older EU Member States, as well as non-EU countries in the Danube region. After dedicating our first year to strategic pilot preparation, our teams are now ready to launch these initiatives. As a kick-off, the 3rd in-person partner meeting and first-year review will be hosted next week by Ludwig Boltzmann Institute in Vienna. ?? What’s on the agenda? ?? Year 1 Highlights: Reviewing progress, key milestones, and lessons learned. ?? Pilot Projects: Discussing methodologies, evaluation frameworks, and data management plans. ?? Knowledge Sharing: Collaborative sessions to refine strategies and share insights. ?? Training & Capacity Building: Launching new materials and finalizing the Knowledge Platform and Integrated Care Management strategy. ?? On-Site Visits: Gaining firsthand insights at the General Hospital of Vienna and ambulance service headquarters. ?? As a caretaker who has been helping a parent manage severe health issues for 20 years, a mom trying to stay on top of my kiddo’s regular doc appointments, and a person who suffers from chronic pain, I feel this acutely. Digital access to healthcare records and a means of engagement should be a given. And while there are patches of access here and there, the full realization of this just “isn’t quite.” But it can and will be. As automation, specialized AI, and Gen AI move toward the incredible possibilities of #agenticAI, I believe we’ll start to have more visibility, control, and greater insights as caretakers of ourselves and others. We’ll have better interactions with care providers who are better equipped to deal with the incredible volume of work that comes with every patient, every day. 'Tackling Chronic Disease with Digitally Integrated Care', part of the ‘Digitally-enabled care pathways’ HIMSS #HIMSS24Europe Track curated by Lisa Rice Duek MBA, RN. If I can share a little story from the session, bear with me: ? I started by asking the audience for a show of hands as answer to: who in the room knows someone with diabetes who HAS / DOESN’T HAVE a longitudinal digital record of their #bloodglucose or #HBA1C, that they can access online to help them understand how they are managing their condition long-term (I flagged that I am not referring to being able to see the last 12h data of their glucose biosensor, that then disappears, but longer trends). ??♀? Guess what; from the 25 people who raised hands in total, only 2 were to acknowledge having this capability, and the rest to flag that, No, patients DON’T have access to long-term trend data. ?? This on the most common of indicators for the most exemplar of chronic conditions used to pilot innovations in most healthcare systems! ???? This tells me, that, while we take it for granted that digital access and means of engagement between patients and the healthcare system would be a given, in reality, it isn’t quite, with a lot of piecemeal examples, pilots, implementations specific to a hospital or a region or a condition… Enough about me, and onward with the expert speakers: ? Andreas Dam presented on one of the innovations he developed for better patient #SelfManagement using digital, in particular YouthCancerForces (UngeKr?fter), a digital companion for young patients battling cancer, which will be nationally implemented as standard care throughout Denmark. ? Olga Vishnevetsky?is currently the Lead Nurse Care Coordinator for the ADLIFE Project in Samson Assuta Ashdod University Hospital. Olga presented on this #EUHorizon2020 - funded project, aiming to improve the quality of life and independence of elderly patients with advanced chronic conditions through digitally supported interventions. Central to the digital tools the project has developed and Olga is testing is the concept of #SharedDecisionMaking and integrated #CarePlanning. ? Day 2 at Radical Health Festival Helsinki closed off with a keynote on the application of precision and prevention at scale, co-designed with the European Society of Cardiology and hosted by Dr. Charles Alessi. We also attended a fascinating panel discussion on pioneering AI advances in cardiology ??in which VEIL.AI’s Juha Paakkola participated. Here are our key takeaways: ?? Nightingale Health CEO Teemu Suna pointed out that Healthcare spending is 10% of GDP in many European countries, and approximately 20% of GDP in the US. Overburdening of healthcare systems results in unequal service, creating a negative feedback loop. The solution is prevention, which we are doing in a way that does not scale— radical new technologies can change this. ?? Leroy Hood, CEO and Founder of Phenome Health presented the ground-breaking new Human Phenome Initiative, which he described as the next Human Genome Project. The Human Phenome Project aims to shift healthcare from its current disease focus to wellness and prevention healthcare by using data-driven health for individuals. ??Antonio Estrella of Talossia and venture partner at Verge HealthTech Fund pointed out that we are tackling the “four horsemen” of the medical apocalypse— coronary artery disease, diabetes, cancer, and Alzheimer’s— at the wrong end of the time scale. The future of health will shift to earlier interventions, before peak health, and influencing behavior for self-change starts with awareness. ??Gerhard Hindricks, Chair of the ESC Digital Health Committee, points out that it is our responsibility to provide the best health and best quality and quantity of life for people. Focusing on the globally leading cause of death, cardiovascular diseases, he outlined how there is a need for a radical change in strategy from intervention to prevention that will be enabled by digital solutions. ?? On October 3rd, 2024, I had the privilege of organizing PREVENTABLE's Midterm Conference and PrevenTalk 2 with the team in Rouen, France. It was Co-organized by i3S - Instituto de Investiga??o e Inova??o em Saúde alongside CHU de Rouen, Centre Henri-Becquerel, SPI - Sociedade Portuguesa de Inova??o, and EHMA - European Health Management Association, this two-day hybrid event brought together researchers, clinicians, policymakers, and stakeholders to discuss the latest findings and future strategies for managing Rare Tumour Risk Syndromes (RTRS). ?? During the Midterm Conference, participants reviewed the project's progress, addressed ongoing challenges, and set the course for its second half. In addition, Keynote speakers and panellists shared in-depth knowledge on hereditary cancer syndromes, the cost-effectiveness of prevention strategies, and effective communication in healthcare, sparking lively discussions and collaborative problem-solving. ?? In PrevenTalk 2: The session focused on the Strengths, Weaknesses, Opportunities, and Threats (SWOT) analysis of RTRS care pathways. Data collected from interviews with top clinicians across Europe highlighted the need for multidisciplinary approaches and personalized care teams in RTRS management.
