Excellent article by Ben Allen. TY for sharing the learnings from a system I have always looked to as a north star. "They did a 1 year public engagement to understand the needs of the community." This should be the starting point of any system re-design. Understanding the needs of the community & people being served ? People-centred design - not system-centred design = greater chance of success + sustainability.

Eurofound continues discussing working conditions in long-term care with stakeholders at the CARE4CARE European Meeting in Brussels ???? today, and how advances can be made, within the context of the EU Care Strategy, implementation of which is key to address challenges faced by the EU. This rapidly growing sector is marked by staff-shortages, and sector-specific physical and, in particular, mental working condition challenges. I'm leaving on a jet plane .......................... don't know when I'll be back again. Leaving the NHS for 10 days to work in Singapore. Great to be with colleagues from Singapore University of Social Sciences (SUSS) launching the Intercontinental Alliance for Integrated Care a new global learning network looking at Ageing and Integration. I'll also be attending the second Asia Pacific Social Prescribing Conferance with colleagues from SingHealth . I've been reflecting on the current state of the NHS and colleagues who may be leaving. Many GPs of my age are leaving the NHS and like the song they are torn to be leaving the NHS but life outside is more attractive. GP Retention is an urgent issue as highlighted by the Royal College of General Practitioners with the current workforce plan for General Practice looking bleak over coming years. This editorial in the BMJ refers to a paper linking retention of acute staff doctors and nurses to mortality but also recognises the importance of continuity in General Practice and retaining the workforce. https://lnkd.in/e2-hJfti We need some urgent action to recruit new GPs and to retain others. Improved working conditions flexiblity in hours and specific programmes are needed urgently as part of the solutions to fix the NHS. In order to deliver the 3 government shifts we need GPs to stay and not to shift ! So I shall be back again returning to the NHS in 10 days time but how can we keep more GPs in the NHS. Today is National Healthcare Decisions Day and since we are all mortal, why not start a conversation with someone close to you about what kinds of care you want and don't want when/if you are seriously ill. This doesn't have to be only the "Who is my health care proxy and do I want to be resuscitated" conversation, though that's super important. Consider also: What are the tradeoffs you think you'd want to make in terms of pain control and alertness? Do you find it comforting to have people around, or would you prefer to be alone more often? What kinds of touch, sounds, scents are comforting to you? Are there questions you need to clarify with a health care professional? When my mother was dying I was surprised to find out that IV fluids were a form of resuscitation. She'd received those routinely when dialysis would result in too much fluid being removed from her body. Suddenly, despite her having had a "do not resuscitate order" and receiving fluids in the past, we were told that fluids now would constitute resuscitation. My point is that it's important to clarify what you are and are not saying "no" to. If you already have an advance directive, start a conversation about whether it needs to be updated. Is there anything you want to add to it? These are conversations it's easy to put off when you're well, but they're not conversations you want to initiate when the need is upon you, so why not make today the day! Need some tools? Dementia is the leading cause of death in the UK, responsible for 1 in 10 of all deaths. Yet it doesn't get the focus that it so urgently needs. I was invited on to Sky News yesterday to discuss this and the impact of dementia on our health and social care system. Out today A new report from CLES and The King's Fund, from work supported by The Health Foundation, looks at how English devolution is working to address health inequalities in our regions and what the govt needs to do to maximise impact. Investigators looked at what's holding back mayoral combined authorities in supporting the best outcomes for everyone at the moment and found the big 3??: ?? the framing of devolution deals ?? accountability and governance ?? funding Looking at the excellent practice happening in CAs, the research proposes three priority areas for the government to enable them to do more - thinking beyond growth, raising expectations of joint working and bringing forward fairer funding.
