???Taking a Proactive Approach: Understanding Medication's Impact on Cognition?? ?? What’s the best outcome I aim for in a workshop? It’s simple: that participants walk away empowered to use the tools and knowledge they’ve gained and apply them. One of the biggest challenges I see? Medication. Now, I’m not a doctor, and I have nothing against medication when necessary, but I know some medications can have side effects on cognition, whether short-term or long-term. Before starting any medication, it’s essential to have an open conversation about the risks vs. benefits. Yet, this conversation rarely happens. In fact, I’ve yet to meet a workshop participant whose GP or pharmacist has proactively discussed the potential cognitive impact of their medication with them. And that’s why I am such a strong advocate for health literacy. No one knows your medical history as well as you do—so it’s vital to ask the right questions and stay vigilant. Just recently, a participant reached out after a workshop, saying: "Michelle, you won’t believe it—three out of five of my medications have a cognitive impact." He was shocked, but he booked an appointment with his pharmacist to review his medications and discuss the cognitive effects. I was delighted to hear it! The takeaway? How do you handle challenging patients? “Challenging” meaning depressed/flat affect, passive aggressive, won’t stop telling what the last doctor said, mad that no one has given them answers - IYKYK ?? I was told by a colleague once, “Vanessa, when it comes to challenging patients, it’s like a barbed wire fence and then it’s like you lay down on the fence and then we all walk across on your back! Thank you!” ?? Here are some steps I commonly take but I’d love to hear what you do/how you handle these patients! 1. Take your time: posture and body language convey that attention is on them. I try not to fidget, crack knuckles, doodle on my paper. 2. Listen: even though I may have the diagnosis already figured out or a number of specific questions I want to ask, first I listen. I believe a big part of a happy patient is that they feel heard. 3. Encourage: point out good/normal things with their exam and then ask a question. “You are moving the wrist well. Does that hurt?” Or something like “It sounds like you have already tried a lot of things to help with the pain.” 4. Teach: explain anatomy or the diagnosis/DDX using analogies the patient will understand BUT do not attempt to explain everything. I usually use a phrase like “We are going to start putting together the pieces of the puzzle as we order these tests.” * Empathy is not a step but is woven in throughout all this interaction. * Avoid implying or saying “This is all in your head.” Though we may think it, it is our duty to figure out if there something we can treat or help with.
