The US FDA's recent approval of a specialized heart stent for infants and children born with congenital heart disease marks a significant milestone in pediatric cardiology! This innovative device, designed to grow with the child as they age, has the potential to revolutionize the treatment of congenital heart defects, reducing the need for multiple invasive surgeries throughout childhood. The Minima stent, developed by Renata, offers a minimally invasive alternative to open-heart surgery. The stent is specifically engineered to treat coarctation and stenosis, common congenital heart conditions in infants and young children. Unlike traditional stents that require replacement as the child grows, the Minima stent can be adjusted with a minimally invasive procedure through a blood vessel in the groin, significantly reducing the recovery time compared to open-heart surgery. My #MotivationMonday starts with what gets me going everyday, my two sons. My oldest was born with a congenital heart defect called Truncus Arteriosus, requiring multiple surgeries and interventions over a lifetime. Bringing pediatric innovation to the forefront isn't just good business. For me, it's personal. I was fortunate to catch Kayleen Brown’s interview panel at DeviceTalks West 2023 discussing the unique complexities and challenges of #pediatric device development. Her article linked below is a succinct summary of important points when assessing this growth market. Greatest challenges: - Miniaturization, with growth potential (babies hearts are the size of a walnut, growing to the size of a hand by 18 years old) -Durability, with safe, effective biomaterials -Safety consideration beyond standard (neurodevelopmental, excessive radiation exposure, vascular preservation) Greatest opportunities: - The growth of a hybrid OR, where surgery and interventions can happen together, is positive momentum for treatment -Expanded focus into #mitral, #tricuspid and #pulmonary valve work is only elevating future possibilities for the youngest patients There is so much great work to be done in our industry. Concepts of Cardiac MR-Imaging and its Impact on Interventional Catheter Procedures in Children with Congenital Heart Defects by Oliver Rompel, M.D., and Thomas Simmerl, M.D. (Universit?tsklinikum Erlangen, Germany) With a prevalence of 1% of all live births, CHDs are the most common congenital abnormalities worldwide, often requiring treatment in early childhood. Detailed knowledge of complex congenital pathologies is essential to #pediatric cardiologists for clinical workup, and to pediatric cardiac surgeons for selecting the most suitable type of surgical access and the required technique of surgical repair. #MRI is an excellent diagnostic tool for detecting a wide range of pediatric diseases. This is particularly true for comprehensive imaging diagnosis of complex congenital heart defects (#CHD). Today, about 85% of children with CHDs survive into adulthood. Consequently, cardiac MRI (CMRI) is also fundamental for follow-up diagnostics in adults with CHDs. Moreover, CMRI is playing an increasingly important role in interdisciplinary image fusion. Next week, the 61st Annual Meeting of the Society for Pediatric Radiology (GPR) will be held in Erlangen, Germany. Patient- and surrogate-reported outcomes are increasingly recognized as important and historically limited dimensions of satisfaction with medical care. We identified caregiver satisfaction with the neurosurgical management of pediatric movement disorders as a key quality metric and report our results across an institutional experience. We found that caregivers were overwhelmingly satisfied with the neurosurgical care for PMDs and would recommend the functional pediatric neurosurgery program to others, and that there was a perception disparity between caregiver- and hospital-identified complications. I spent a good day at Duke's pediatric ECHO lab. Everyone has been so helpful and knowledgeable in teaching me pediatric pathology workups and I have to say, a whole other world, compared to adult ECHO. I really learned and seen a lot in one day. In this episode, host Ashley Agan, MD MBA discusses eustachian tube (ET) dilation with Dr. Dennis Poe, a neurotologist and Professor of Otolaryngology at Harvard Medical School. First, the surgeons describe clinically meaningful differences between adult and pediatric ETs. Then, Dr. Poe reviews workup of pediatric ET disorders. Since inflammation accounts for most ET dysfunction, controlling comorbid allergies and/or performing adenoidectomy provides relief to some patients. For patients with continued symptoms, balloon dilation – now approved for children as young as eight years of age– directly counteracts obstructive inflammation in the tube lumen. ?? Cerebral oxygenation saturation in childhood: difference by age and comparison of two cerebral oximetry algorithms ?? byY Kubo et al. from Hokkaido University ???? A groundbreaking study has for the first time, explored regional cerebral oxygen saturation (rSO2) across a broad age range of pediatric patients undergoing non-cardiac surgery, offering invaluable insights into pediatric perioperative care. ?? Study Highlights: Objective: To assess and compare rSO2 levels in infants, toddlers, and schoolchildren during non-cardiac surgeries using two different oximeters: tNIRS-1 (time-resolved spectroscopy) and INVOS 5100C. Participants: Seventy-eight children, evenly divided among infants, toddlers, and schoolchildren. ?? Significant variation in rSO2 levels across different age groups. INVOS 5100C reported significantly lower rSO2 in infants, while tNIRS-1 showed higher values in toddlers. Moderate correlation between the two oximeters, with INVOS readings being about 20% higher than those from tNIRS-1, indicating possible discrepancies due to algorithm differences. rSO2 values were influenced by physiological factors such as blood pressure and sevoflurane concentration. ?? Implications: This study underscores the complexity of monitoring cerebral oxygenation in pediatric patients and highlights the need for age-specific considerations in perioperative care. Rezolute, Inc. announced positive topline results from the Phase 2 clinical study of RZ402, the first oral therapy to demonstrate reduction in Diabetic Macular Edema (DME) in patients who are na?ve to or have received limited anti-vascular growth factor (anti-VEGF) injections. "These data are very encouraging and are supportive of the potential for a new first-line, non-invasive treatment for DME,” said Quan Dong Nguyen, MD, MSc, FAAO, FARVO, FASRS, Professor of Ophthalmology at the Byers Eye Institute, and Professor of Medicine and Professor of Pediatrics at the Stanford University School of Medicine, and a member of Rezolute’s Scientific Advisory Board. These results showcased both good safety profiles and a reduction in central subfield thickness (CST). Exploring Variation in Antegrade Colonic Enema (ACE) Channel Placement Practices We are excited to share the findings of our recent multicenter study on the practice patterns surrounding the Antegrade Colonic Enema (ACE) channel placement conducted with the Pediatric Colorectal and Pelvic Learning Consortium (PCPLC). We aimed to understand the variations in practice and patient characteristics for this effective management option for defecation disorders. Our study, which included data from 500 patients treated between 2017 and 2022, revealed significant variations in practice across different centers. Here's a snapshot of our key findings: 1. Age at Surgery: The median age at ACE channel placement was 7.6 years for the overall cohort, with significant variations across PCPLC centers (p = 0.009). At PCPLC centers, the median age was 8.1 years. 2. Types of Channels: - Malone Appendicostomy: 74.2% of patients (most common). - Cecostomy: 23.2% of patients. - Neo-Malone Appendicostomy: 2.6% of patients. - Median age varied slightly by channel type. 3. Indications for Placement: - Cecostomy: Mostly for idiopathic/refractory constipation (52.6%). - Malone and Neo-Malone Appendicostomy: Commonly indicated for anorectal malformations (47.2% and 61.5%, respectively). 4. Institutional Preferences: The four highest volume centers preferred Malone appendicostomy over cecostomy, highlighting the variation in preferred initial channel types across institutions. Our study's highlights the need for a more standardized approach to ACE channel placement. This approach, tailored to diagnosis and patient age, can help us better understand patient-centered outcomes and refine our recommendations. Ultimately, it ensures that each patient receives the most appropriate care, potentially improving their quality of life.
